研究目的：

评价经粪菌移植治疗糖尿病周围神经病变的疗效及安全性。  

Objectives of Study：

To evaluate the efficacy and safety of fecal microbiota transplantation in the treatment of diabetic peripheral neuropathy.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1)1型或2型糖尿病患者；
(2)年龄在18-70岁；
(3)糖化血红蛋白<11%；
(4)至少有三个月的周围神经病变病史（表现为局部或肢体远端麻木、疼痛、蚁走感、袜套感等）；
(5)神经电生理及功能评分量表至少存在1项异常：NCV减慢，VPT> 25伏特，TCSS评分>5分或VAS >5分。

Inclusion criteria

1. Type 1 or type 2 diabetes;
2. Aged 18-70 years old;
3. HbA1c<11%;
4. At least three months of history of Diabetic Peripheral Neuropathy (including local or distal numbness, pain, ant walking sensation, garter sensation, etc);
5. At least one abnormality in the neuro electrophysiological and functional scale:NCV slows down, VPT> 25 volts, TCSS score >5 points or VAS score >5 points.

排除标准：

（1）持续性呕吐或怀疑有消化道梗阻者；
（2）胃切除术、胃底折叠术、结肠造瘘等消化系统外科手术；
（3）接受肠内营养或鼻饲营养或其他营养灌注方式；
（4）过去3个月内使用过抗生素，并持续3天或以上；
（5）中、重度肾功能损伤（血肌酐＞2mg/dL或177mmol/L），或肝功能异常（ALT>2倍正常值上限）的患者；中、重度慢性阻塞性肺部疾病；重度高血压患者、脑血管意外患者；
（6）伴有可能引起周围神经病的相关疾病，如骨关节炎、颈腰椎疾病、结缔组织病、外周血管疾病、副肿瘤性周围神经病、带状疱疹感染、甲状腺功能异常、重度营养不良者；
（7）曾服用可引起周围神经病的药物，如异烟肼、痢特灵等，合并有严重心脑血管、肝、肾及造血系统疾病；
（8）患有严重器质性疾病，如癌症、冠心病、心肌梗塞以及中风等；
（9）患有肺结核和艾滋病等传染病；
（10）发生严重糖尿病并发症的患者，如：糖尿病酮症、酮症酸中毒、乳酸性酸中毒、糖尿病高渗状态、糖尿病足、严重视网膜病等；
（11）酗酒（1周饮酒5次以上，平均每次2两白酒或半斤黄酒或5瓶啤酒以上）；
（12）过去3个月内因下列疾病接受药物治疗的：胆囊炎、消化道溃疡、尿路感染、急性肾盂肾炎、膀胱炎、甲亢等甲状腺功能异常；
（13）妊娠期以及哺乳期妇女；
（14）贫血：血红蛋白<10g/dl；
（15）肢体残疾，或其他原因引起的生活不能自理、不能清楚地回忆、回答问题者，或有明显的运动障碍；
（16）不能保证充足的时间参与本项目的。

Exclusion criteria：

1. Persistent vomiting or suspected gastrointestinal obstruction;
2. Gastrectomy, fundoplication, colostomy and other digestive system surgery;
3. Receiving enteral nutrition or nasal feeding or other nutrient infusion;
4. Antibiotics used in the past 3 months for 3 days or more;
5. Patients with moderate or severe renal impairment (> 2mg/dL or 177mmol/L) or liver dysfunction (ALT> 2-fold upper limit); Moderate to severe chronic obstructive pulmonary disease;Patients with severe hypertension and cerebrovascular accident;
6. May cause peripheral neuropathy related diseases, such as osteoarthritis, cervical lumbar diseases, connective tissue disease, peripheral vascular disease, tumor peripheral neuropathy, herpes zoster infection, abnormal thyroid function, severe malnutrition;
7. Have taken drugs that can cause peripheral neuropathy, such as isoniazid, dilorazepam, etc., with severe cardiovascular and cerebrovascular diseases, liver, kidney and hematopoietic system diseases;
8. With severe organic disease, e.g. cancer, coronary heart disease, myocardial infarction and cerebral apoplexy;
9. With infectious disease, e.g. phthisis and Aids;
10. With severe diabetes complications, e.g. leg ulcers, severe liver and kidney damage, severe neurological diseases and severe retinopathy;
11. Intemperance (drinking more than 5 times in one week and drinking more than 100g spirit, 250g rice wine or 5 bottles of beer);
12. Received drug therapy aim at the following diseases, e.g. cholecystitis, peptic ulcer, urinary tract infection, acute pyelonephritis, urocystitis or hyperthyreosis;
13. Pregnancy or planning to become pregnant;
14. Anemia: HbA1c less than 10g/dl;
15. Physical Disability or self-care disability or disability of recalling clearly and answering questions caused by any other reasons;
16. Without enough time taking part in this project.
