研究目的：

心肌梗死所致的细胞丢失和瘢痕形成是影响心功能的主要因素，也是导致顽固性心力衰竭乃至死亡的病理基础。对于各种顽固性心衰，内科治疗效果很差，患者有极高的死亡率。心肌梗死的治疗，包括药物治疗、介入治疗和手术治疗等虽能够改善心肌缺血和心衰的症状，使闭塞的血管再通，却不能替代不可复生的坏死心肌。而心脏移植虽能替代受损的心脏，但供体缺少，费用高，临床难以推广。由于冠心病的高发病率、高死亡率对人类健康的危害性，此研究主要目的就是寻找一种能够替代坏死心肌组织，改善心脏功能的新方法。  

Objectives of Study：

The cell loss and scar formation caused by myocardial infarction are the main factors affecting heart function and the pathological basis for intractable heart failure and even death. For all kinds of intractable heart failure, medical treatment is very poor, and patients have a very high mortality rate. The treatment of myocardial infarction, including drug therapy, interventional therapy, and surgical treatment, can improve the symptoms of myocardial ischemia and heart failure and recanalize the occluded blood vessels, but it cannot replace the irreversible necrotic myocardium. Although heart transplantation can replace a damaged heart, the lack of donors and high costs make it difficult to promote clinically. Due to the high incidence and mortality of coronary heart disease which are harmful to human health, the main purpose of this study is to find a new method that can replace necrotic myocardial tissue and improve heart function.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1．严重充血性心衰NYHA Class II/Ⅲ。
2. 心脏CT影像显示陈旧性心肌梗死伴心室壁局部活动障碍的患者。
3. 射血分数≤45%。
4. 病人在手续前必须签署书面的知情同意书。
5. 有CABG手术适应症。

Inclusion criteria

1. Severe congestive heart failure NYHA Class II/III;
2. Heart CT images show patients with old myocardial infarction and local ventricular wall dysfunction;
3. Ejection fraction <=45%;
4. The patient must sign a written informed consent before the procedure;
5. There are indications for CABG surgery.

排除标准：

1. 年龄 ＞75岁。
2. 最近（4周内），急性冠状动脉综合征病史（如，MI Q波和、或非Q波），因不稳定型心绞痛或脑血管综合症（发作）而住院。
3. 有持续心室性心动过速或颤动的历史记录。
4. 患者有植入式除颤器（ICD）。
5. 合并其他病变如瓣膜性心脏病（主动脉瓣、 二尖瓣病变需要瓣膜置换术等）。
6. 异常凝血酶原或部分凝血活酶时间或血小板减少症（血小板计数 ＜50,000）。
7. 严重的肾功能不全（如，血尿素氮＞50mg/dL、肌酐2.5 mg/dL）或肝脏疾病（如，SGOT/AST or SGPT/ALT＞3 x ULN）。
8. 有毒品接触史。
9.下列检查阳性病人：人体免疫缺陷病毒 (HIV)、 B 型肝炎病毒 (HBV)，和/或丙型肝炎病毒 (HCV)、梅毒（RPR）、巨细胞病毒（CMV-IgM）。
10.有骨骼肌病史，如肌病。
11.正在怀孕或哺乳或任何具有生殖能力不愿在治疗期间控制生育的女性病人。
12.并发感染。
13.根据调查员的判断，任何对方案的执行、对病人的安全和会干扰肌母细胞移植治疗效果的主要疾病。

Exclusion criteria：

1. Age ＞75 years old.
2. Recently (within 4 weeks), a history of acute coronary syndrome (eg, MI Q wave and/or non-Q wave), hospitalization due to unstable angina or cerebrovascular syndrome (onset).
3. There is a history of continuous ventricular tachycardia or fibrillation.
4. The patient has an implantable defibrillator (ICD).
5. Combined with other diseases such as valvular heart disease (aortic valve, mitral valve disease requires valve replacement, etc.).
6. Abnormal prothrombin or partial thromboplastin time or thrombocytopenia (platelet count <50,000).
7. Severe renal insufficiency (eg, blood urea nitrogen>50mg/dL, creatinine 2.5mg/dL) or liver disease (eg, SGOT/AST or SGPT/ALT>3 x ULN).
8. History of drug exposure.
9. The following positive patients: human immunodeficiency virus (HIV), hepatitis B virus (HBV), and/or hepatitis C virus (HCV), syphilis (RPR), cytomegalovirus (CMV-IgM).
10. Have a history of skeletal muscle diseases, such as myopathy.
11. Female patients who are pregnant or breastfeeding or have reproductive capacity who are unwilling to control birth during treatment.
12. Concurrent infection.
13. According to the judgement of the investigator, any major diseases that will interfere with the therapeutic effect of myoblast cell transplantation in the implementation of the plan, safety to the patient.
