研究目的：

优化和建立一种无创、全面、快速、低成本的Hp鉴定与耐药性分析的30余重基因检测系统（non-invasive Hp Identification and Drug Resistance Multiple Genetic Detection System，non-invasive Hp-IDMG），规范样本采集、运输、检测、报告等检测流程，并在本市内推广应用，以提升全市Hp的诊疗水平和能力。  

Objectives of Study：

Optimize and establish a non-invasive, comprehensive, rapid and low-cost Hp identification and drug resistance analysis of more than 30 multiple genes detection system (non-invasive Hp Identification and Drug Resistance Multiple Genetic Detection System, non-invasive Hp-IDMG). Standardize the testing procedures of sample collection, transportation, testing and reporting, and popularize and apply them in this city to improve the level and ability of Hp diagnosis and treatment.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）2020年10月~2022年9月来华东医院消化科门诊并进行胃镜检查、胃黏膜活检和13C-呼气试验的消化不良症状的患者
（2）年龄不限，男女不限。
（3）消化不良诊断的采用2007年中华医学会消化分会胃肠动力组的定义：临床表现为上腹部疼痛或烧灼感、餐后上腹饱胀和早饱感的症候群,可伴食欲不振、嗳气、恶心或呕吐等症状。

Inclusion criteria

（1）Patients in Huadong Hospitals Gastroenterology Clinic from October 2020 to September 2022 who had dyspeptic symptoms and undergone gastroscopy, gastric mucosal biopsy and 13C-breath test;
（2）There is no restriction on age and gender;
（3）The diagnosis of dyspepsia is based on the definition of the gastrointestinal motility group of the Digestive Branch of the Chinese Medical Association in 2007: clinical manifestations are symptoms of pain or burning in the upper abdomen, fullness of the upper abdomen after a meal, and early satiety, which may be accompanied by loss of appetite, belching, and nausea Or vomiting and other symptoms.

排除标准：

（1）内镜证实有活动性出血者；
（2）已接受过Hp根除治疗者；
（3）最近1个月曾经服用铋剂、质子泵抑制剂、H2受体拮抗剂以及抗生素者；
（4）曾对本次治疗所用的药物有过敏史或皮试阳性患者；
（5）孕期及哺乳期患者。

Exclusion criteria：

（1）Patients with active bleeding confirmed by endoscopy;
（2）Patients who have received Hp eradication therapy;
（3）Patients who have taken bismuth, proton pump inhibitors, H2 receptor antagonists and antibiotics in the past month;
（4）Patients with a history of allergies or a positive skin test during this treatment;
（5）Patients during pregnancy and lactation.

诊断试验：

Diagnostic Tests：

金标准或参考标准（即可准确诊断某疾病的单项方法或多项联合方法，在本研究中用于诊断是否有该病的临床参考标准）：

(1)幽门螺杆菌鉴定参考标准（多项联合）： 1. 幽门螺杆菌体外培养+显微镜检查，观察到典型幽门螺杆菌形态。 2. C13-呼气试验：按照《C13-尿素呼气试验的操作规程》的流程和步骤进行检测。 3. 组织病理染色镜检：由病理科专业的资深技师染色镜检和诊断。 (2)幽门螺杆菌耐药性检查参考标准： E-test法体外药敏试验，读取M-H培养基上抑菌圈与E-test纸条相交处的数值即为待检Hp对该药物的最低抑菌浓度（MIC）。根据美国临床和实验室标准协会（CLSI）2013年版及相关文献的判断标准，判断耐药的MIC值为：克拉霉素MIC ≥ 1 μg/mL、甲硝锉MIC ≥ 8 μg/mL、喹诺酮类MIC ≥ 1 μg/L，阿莫西林MIC ≥ 8 μg/mL。

(1)Reference standard for the identification of Helicobacter pylori (multiple combinations): 1. Helicobacter pylori in vitro culture combined with microscopic examination. 2. C13- Breath Test. (2)Reference standard of Helicobacter pylori drug resistance examination: In vitro drug susceptibility test (E-test).
