研究目的：

在行体外循环手术的患者中，结合其术后相关临床表现，运用代谢组学对出现术后谵妄的患者和未出现术后谵妄的患者血浆相关代谢产物进行分析，并对术后谵妄患者进行出院后远期认知随访，评估其是否转为术后神经认知障碍，以及分析相应差异代谢产物的变化。借助生物信息学工具，旨在探讨体外循环手术术后神经认知障碍发生的相关分子机制，以期为其提供具有意义的新型临床生物学标记物以及发现后的早期干预。  

Objectives of Study：

In extracorporeal circulation operation patients, combined with its related clinical manifestations, using the metabonomics to appear postoperative delirium and does not appear in patients with postoperative delirium in patients with plasma were analyzed, and related metabolites and the postoperative delirium in patients with long-term cognitive follow-up after discharge, assess its to postoperative neural cognitive impairment, and analyzing the change of the corresponding differences metabolites. With the help of bioinformatics tools, this paper aims to explore the molecular mechanism of neurocognitive impairment after extracorporeal circulation surgery, so as to provide significant new clinical biological markers and early intervention after discovery.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1） 手术类型：体外循环下心脏手术（冠脉搭桥术或瓣膜置换术）;
2） 年龄18-75岁，性别不限;
3） 无酒精成瘾史，或近两周内没有饮酒史
4） 美国纽约心脏病学会( NYHA)心功能分级Ⅱ或Ⅲ级，左室射血分数( EF) ＞50%
5） 术前白细胞计数正常
6） 术后入住ICU
7） 计划在手术后住院>24小时

Inclusion criteria

1) Type of surgery: extracorporeal circulation cardiac surgery (coronary artery bypass grafting or valve replacement);
2) Ages 18-75, regardless of gender;
3) No history of alcohol addiction, or no history of alcohol use in the last two weeks
4) the New York heart association (NYHA) heart function class Ⅱ or Ⅲ, left ventricular ejection fraction (EF) > 50%
5) Preoperative white blood cell count was normal
6) Admitted to ICU after surgery
7) plan to be hospitalized for > for 24 hours after surgery

排除标准：

1） 拒绝参加；
2） 既往有心脏手术史或二次心脏手术史；急性心包填塞和心肌梗死、严重窦性心动过缓、房室传导阻滞或病态窦房结综合征；
3） 既往患有肺部慢性疾病，未吸氧下脉搏氧饱和度<90%；
4） 肝功能Child-Pugh C级，严重肝功能障碍；
5） 严重肾功能不全，需术前进行肾脏替代治疗；
6） 围手术期胰岛素治疗
7） 中风，精神分裂症，抑郁症，帕金森病，癫痫或痴呆；
8） 蒙特利尔量表（MoCA）≤25分，若受试者受教育年限≤12年，在测试结果上加1分；
9） 医院用焦虑抑郁量表（HADS）≥8分（见附件3）；
10） 文盲，语言障碍或重大听力或视力损害而无法进行交流；

Exclusion criteria：

1) Refuse to participate;
2) Previous history of heart surgery or secondary heart surgery; Acute cardiac tamponade and myocardial infarction, severe sinus bradycardia, atrioventricular block or pathological sinoatrial node syndrome;
3) Previous chronic pulmonary disease, pulse oxygen saturation <90% without oxygen;
4) Child-pugh grade C liver function, severe liver dysfunction;
5) Severe renal insufficiency requires preoperative renal replacement therapy;
6) Perioperative insulin treatment
7) Stroke, schizophrenia, depression, Parkinson's disease, epilepsy or dementia;
8) Montreal Scale (MoCA) ≤25 points; if the subject's years of education ≤12 years, 1 point will be added to the test result;
9) Hospital anxiety and depression Scale (HADS) ≥8 points ;
10) Inability to communicate due to illiteracy, language impairment or major hearing or visual impairment;

The Delirium assessment scales (ICU-CAM and CAM scales) were used to diagnose postoperative delirium. Montreal Scale (MoCA) was used to diagnose delayed neurocognitive recovery.
