研究目的：

探索减少分娩期会阴损伤及促进产后盆底功能康复的有效方法。  

Objectives of Study：

To explore effective methods to reduce perineal injury during childbirth and promote the recovery of pelvic floor function after childbirth.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）自愿参加（签署知情同意）；（2）初产妇；（3）单胎；（4）头位；（5）孕周：34±6周（根据末次月经时间和早孕期超声核对孕周，下文中孕34周均指孕34±6周）；（6）预产期年龄：18～35岁（不包括35岁）。

Inclusion criteria

(1) Voluntary participation (signing informed consent);(2) Primipara;(3) Single fetus;(4) Head;
(5) Gestational week: 34±6 weeks;(6) Expected age of delivery: 18-35 (excluding 35).

排除标准：

（1）内外科合并症：如糖尿病合并妊娠、慢性高血压合并妊娠、妊娠期高血压（需药物治疗）及合并脏器功能不全者。（2）早产高风险：①感染因素：入组时患细菌性阴道病、阴道假丝酵母菌病及滴虫感染、生殖器疱疹等；②宫颈机能不全：宫颈环扎术后、晚期流产史、宫颈进行性缩短；③羊水过多或羊水过少者；④胎膜早破；⑤胎盘异常：前置胎盘、胎盘绒毛膜血管瘤、胎盘植入、膜状胎盘 (帆状胎盘、球状胎盘)等；（3）有其他阴道分娩禁忌症：生殖道严重畸形、重度漏斗骨盆等；（4）慢性便秘史、慢性咳嗽史，妊娠前生殖器脱垂、尿失禁等；（5）肌肉疾病如重症肌无力等，结缔组织疾病如红斑狼疮、类风湿关节炎、硬皮病、皮肌炎等；（6）既往会阴及肛门区手术史；（7）外阴病变：外阴白斑、外阴静脉曲张；（8）精神异常；（9）胎儿发育异常：宫内生长受限；胎儿侧脑室增宽；胎儿长骨偏短等。

Exclusion criteria：

(1) Who with physical and surgical disease; (2) High risk of premature delivery;(3) Other contraindications for vaginal delivery;(4) Chronic constipation history, chronic cough history, genital prolapse before pregnancy, urinary incontinence, etc.;(5) Muscular diseases such as myasthenia gravis, connective tissue diseases such as SLE,etc.;(6) Previous perineal and anal surgeries;(7) Vulva lesions: vulva leukoplakia and varicose veins;(8) Mental disorders;
(9) Abnormal fetal development.
