研究目的：

生酮饮食联合奥氮平对于精神分裂症患者代谢紊乱和认知症状的改善作用。  

Objectives of Study：

To study effect of ketogenic diet combined with olanzapine on metabolic disorder and cognitive symptoms in schizophrenic patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

年龄18-55岁患者，男女不限；
符合DSM-V精神分裂症诊断标准；PANSS评分≥60；
稳定服用奥氮平至少6个月（至少1个月之内剂量未变）
合用其它抗精神病药物者，至少稳定1个月；
空腹血糖6.1-6.9 mmol/l或糖耐量实验中给予口服75g葡萄糖后两小时血糖>7.8 mmol/l，BMI≥27 kg/m2

Inclusion criteria

1. Patients aged 18-55 years old, male or female;
2. The patients who met the DSM-V diagnostic criteria of schizophrenia; PANSS score ≥ 60;
3. Patients who have been taking olanzapine for at least 6 months (the dose has not changed within at least 1 month);
4. Patients with other antipsychotics were stable for at least one month;
5. Fasting blood glucose of 6.1-6.9 mmol / L or patients with blood glucose > 7.8 mmol / L and BMI ≥ 27 kg / m2 in two hours after oral administration of 75g glucose in glucose tolerance test.

排除标准：

患有严重消化心血管呼吸肝脏泌尿系统或具有生酮饮食禁忌的遗传代谢性疾病（如ETC缺陷中复合物2~5型脂肪酸代谢障碍等）以及先天性免疫缺陷病等不适合生酮饮食治疗者
1型和2型糖尿病人或正在服用抗糖尿病药物
肝功能受损（肝转氨酶>2倍正常值上限）
肾功能受损（肌酐>150?M）
胰岛功能受损（急性或慢性胰腺炎）
严重的高血压（收缩压>180mmHg舒张压>100mmHg）
服用减肥药
处于怀孕或哺乳期的女性
物质滥用

Exclusion criteria：

1. Patients with severe digestive, cardiovascular, respiratory, liver and urinary system or genetic metabolic diseases with ketogenic diet taboo (such as complex 2-5 fatty acid metabolic disorder in etc deficiency) and congenital immunodeficiency disease;
2. Patients who are not suitable for ketogenic diet therapy;
3. Patients with type 1 and type 2 diabetes or taking antidiabetic drugs;
4. Patients with impaired liver function (ALT > 2 times the upper limit of normal value);
5. Patients with impaired renal function (creatinine > 150 m);
6. Patients with impaired islet function (acute or chronic pancreatitis);
7. Patients with severe hypertension (systolic blood pressure > 180mmhg, diastolic blood pressure > 100mmhg);
8. Patients taking diet pills;
9. Pregnant or lactating women;
10. Patients with substance abuse.
