基于《内经》“补荥通俞”理论对针刺治疗糖尿病性动眼神经麻痹的疗效观察  

Objectives of Study：

Observation on therapeutic effect of acupuncture on diabetic oculomotor nerve palsy based on the theory of "Buxing Tongshu" in Neijing

药物成份或治疗方案详述：

将 30 例糖尿病性动眼神经麻痹患者随机分为针刺组与 口服西药组，针刺组，局部选取：承泣、四白、下关、太阳、头维；远端选取：内 庭、陷谷、二间、三间、合谷、曲池、足三里、解溪、三阴交（根据患者四诊进行 辨证选取），针刺得气后，留针 20-30 分钟，每周治疗 3 次；口服西药组给予甲钴 胺片，一次 1 片，一日三次。4 周为一疗程，共治疗三个疗程。分别在治疗前、治 疗 4 周、治疗 8 周、治疗 12 周后根据睑裂高度、斜视角度、视觉相关生活质量评分 等评价其治疗效果。治疗 6 个月及 1 年后对患者进行随访，以观察疗效的持续性。  

Description for medicine or protocol of treatment in detail：

Methods: 30 patients with diabetic oculomotor nerve palsy were randomly divided into acupuncture group and oral western medicine group. In the acupuncture group, Chengqi, Sibai, Xiaguan, Taiyang and Touwei were selected locally; the distal part was selected as inner court, sunken Valley, Erjian, Sanjian, Hegu, Quchi, Zusanli, JieXi, Sanyinjiao (according to the four diagnosis and treatment of the patients). After getting Qi by acupuncture, the needles were kept for 20-30 minutes The oral western medicine group was given Mecobalamin tablets, one tablet a time, three times a day. Four weeks as a course of treatment, a total of three courses. Before treatment, 4 weeks, 8 weeks and 12 weeks after treatment, the treatment effect was evaluated according to blepharoplasty height, strabismus angle and visual related quality of life score. The patients were followed up for 6 months and 1 year to observe the persistence of the curative effect. 

纳入标准：

（1）糖尿病诊断成立（符合 1998 年 WHO 提出的关于糖尿病的诊断标准）；
（2）急性起病，有第三脑神经受累表现；
（3）神经电生理检查支持外周神经受累。
（4）所有患者均自愿参加本次临床研究，并签署知情同意书。

Inclusion criteria

(1) The diagnosis of diabetes was established (in accordance with the diagnostic criteria for diabetes proposed by who in 1998);
(2) The third brain nerve was involved in acute onset;
(3) Electrophysiological examination supported peripheral nerve involvement;
(4) All patients voluntarily participated in the clinical study and signed informed consent.

排除标准：

（1）经 CT、MRI 或数字减影血管造影（DSA）检查，诊断为颅内动脉瘤、脑干
血管病变、痛性眼肌麻痹、重症肌无力、颅内肿瘤等其它可引起动眼神经麻痹的疾病
者；
（2）精神病患者；
（3）对针灸刺激有严重的异常反应（如晕针等），不能坚持针灸者；
（4）严重心、肝、肾及凝血功能。

Exclusion criteria：

(1) By CT, MRI or digital subtraction angiography (DSA), the patients were diagnosed as intracranial aneurysms, brainstem vascular lesions, painful ophthalmoplegia, myasthenia gravis, intracranial tumors and other diseases that may cause oculomotor nerve palsy;
(2) Mental patients;
(3) Those who have serious abnormal reaction to acupuncture stimulation (such as dizziness) and can not adhere to acupuncture and moxibustion;
(4) Severe heart, liver, kidney and coagulation function.
