研究目的：

探索补充可发酵膳食纤维能否预防GDM，可发酵膳食纤维对GDM患者血糖调控、肠道菌群变化、SCFAs合成和母婴结局的影响。  

Objectives of Study：

Objective to explore whether the supplementation of fermented dietary fiber can prevent GDM, and the effect of fermented dietary fiber on blood glucose regulation, intestinal flora changes, SCFAs synthesis and maternal and infant outcomes in GDM patients.

药物成份或治疗方案详述：

水溶性可发酵膳食纤维 

Description for medicine or protocol of treatment in detail：

soluble fermentable dietary fiber 

纳入标准：

研究对象：选取2020年10月-2022年5月在我院产科门诊建卡并规律产检的具有GDM高危因素的孕妇。
纳入标准：
1）汉族女性；
2）单胎妊娠；
3）20周前建卡产检；
4）在孕24-28周进行了75g口服糖耐量试验（oral glucose tolerance test，OGTT）、血清胰岛素（insulin）及糖化血红蛋白（hemoglobin A1c, HbA1c）的检测；
5）具有1种及以上下列高危因素：年龄大于35岁，妊娠前超重或肥胖（BMI>25kg/m2），患多囊卵巢综合征，有糖尿病家族史，既往有不明原因的死胎、死产、流产史，有胎儿畸形史，有羊水过多史，有巨大儿分娩史，有GDM病史，本次妊娠发现胎儿大于孕周、羊水过多，反复出现外阴阴道假丝酵母菌病。

Inclusion criteria

Pregnant women with GDM high-risk factors were selected from October 2020 to May 2022 in our hospital obstetric outpatient card and regular prenatal examination.
inclusion criteria:
1) Han women;
2) single pregnancy;
3) establishment of birth card before 20 weeks;
4) 75g oral glucose tolerance test (OGTT), serum insulin and glycosylated hemoglobin (A1C) were conducted at 24-28 weeks of pregnancy, HbA1c);
5) one or more of the following risk factors: older than 35 years old, overweight or obesity before pregnancy (BMI>25kg/m2), polycystic ovary syndrome, family history of diabetes, history of unexplained Intrauterine fetal death, stillbirth and abortion, history of fetal malformation, history of polyhydramnios, history of macrosomia, history of GDM, history of polyhydramnios, history of recurrent vulvovaginal candidiasis and the fetal development is greater than gestational age.

排除标准：

1. 孕前糖尿病（PGDM)：包括妊娠前已确诊为糖尿病，首次产检时空腹血糖≥7mmol/l，首次产检时行75gOGTT餐后2小时血糖≥11.1mmol/l，首次产检时发现患者有典型的高血糖和高血糖危象症状，同时任意血糖≥11.1mmol/l，首次产检时糖化血红蛋白≥6.5%；
2. 行生殖医学手段受孕的产妇，包括且不仅限于体外人工受孕、药物促排卵等手段；
3. 因慢性感染性疾病行长期服用抗生素类药物者。

Exclusion criteria：

1. Pre pregnancy diabetes mellitus (PGDM): including diabetes mellitus diagnosed before pregnancy, abdominal blood glucose ≥ 7 mmol / L during the first prenatal examination, 75g OGTT for the first time, 2-hour postprandial blood glucose ≥ 11.1mmol/l, typical hyperglycemia and hyperglycemia crisis symptoms, and at the same time, any blood glucose ≥ 11.1mmol/l, glycosylated hemoglobin ≥ 6.5% at the time of the first labor examination;
2. reproductive medicine means Pregnant women, including but not limited to in vitro artificial pregnancy, drug ovulation and other means;
3. chronic infectious diseases due to long-term use of antibiotics.
