研究目的：

明确全麻患者术中潮气量大小和呼气末正压通气与非心脏手术后急性肾损伤的关系，为围术期需要机械通气患者预防急性肾功能损伤提供参考和意见建议。  

Objectives of Study：

To clarify the relationship between intraoperative tidal volume and end-expiratory positive airway pressure in patients with general anesthesia and acute kidney injury after non-cardiac surgery, so as to provide references and suggestions for patients who need mechanical ventilation to prevent acute renal injury during perioperative period.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

行非腹腔镜手术、非心脏手术、机械通气时间>90 min的患者，年龄18~80岁，ASA Ⅰ~Ⅲ 级，至少术后在医院时间超过24h。

Inclusion criteria

The inclusion criteria were adults aged 18-80 yr;ASA physical status 1 to 3;scheduled to undergo elective non-cardiac and non-laparoscopic surgery under mechanical ventilation for at least 90 minutes with an anticipated length of stay of at least 24 hours.

排除标准：

(1).手术仅在局部麻醉、区域麻醉或麻醉监护下进行；(2).既往有严重肾脏疾病的患者，慢性肾脏疾病史超过4期或透析或术前估计肾小球滤过率(EGFR)< 30ml/min；(3).需要单肺隔离的手术和肺移植手术；(4).机械通气时间小于90 min；(5).腹腔镜下手术及泌尿外科手术；(6).重度肺通气功能障碍；(7).全身麻醉药物过敏。

Exclusion criteria：

(1)The operation is only performed under local anesthesia or regional anesthesia or anesthesia supervision;(2). Patients with serious renal dyfunction,a history of chronic kidney disease of more than 4 stages or undergoing dialysis before surgery or preoperative estimation of EGFR＜30ml/min.(3) Lung transplantation and surgery requiring isolation of one lung;(4). Mechanical ventilation time is less than 90 minutes;(5). Laparoscopic surgery and urological surgery;(6) severe pulmonary ventilation dysfunction;(7) Allergy to general anesthesia drugs.
