研究目的：

通过使用自体尿源活性细胞外泌体（UDC-Exos），促进尿道下裂尿道海绵体缺损的修复，提高尿道下裂的手术治愈率和减少如尿道瘘、尿道狭窄、阴茎弯曲、尿道憩室等并发症的发生，实现完全的解剖和功能性尿道重建。  

Objectives of Study：

To improve the cure rate and reduce the incidence of complications of hypospadias by using UDC-Exos (Exosomes secreted by urine derived cells) to promote the repair of the corpus spongiosum defect of hypospadias and complete anatomical and functional urethral reconstruction.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

染色体46,XY，需应用包皮皮片或口腔黏膜片作为尿道重建材料、且需行分期手术治疗的重度尿道下裂患儿，监护人知情同意。

Inclusion criteria

Children with severe hypospadias who need to undergo staged operation using autologous prepuce or oral mucosa as urethral reconstruction materials, and their guardians should have informed consent.

排除标准：

DSD（性发育异常），合并其他外生殖器异常，可一期完成尿道下裂手术者，符合纳入标准、但监护人拒绝参加本项研究的患儿。

Exclusion criteria：

Patients with DSD (dysplasia of sexual development). Patients with other external genitalia abnormalities.Patients who could complete hypospadias surgery in one stage. Patients' guardians refused to participate in the study.

第一期手术中在包皮皮片或口腔黏膜片外面涂抹UDC-Exos（浓度为1×1011／ml），然后将包皮皮片或口腔黏膜片固定在阴茎体腹侧表面，促使尿道海绵体缺损的修复，构建带有海绵体的尿道修复材料；第二期6-9个月后用第一期手术构建存活的包皮或口腔黏膜组织完成尿道卷管成形，于术中获取组织进行尿道海绵体成形的组织病理学评估。
