研究目的:

数十年的研究试图通过评估潮气量、呼气末正压和肺复张来确定最佳的通气策略。然而,这些研究得出了混杂的结论。对于潮气量来说已经达成比较一致的结论,低潮气量通气较高潮气量通气更能改善肺功能。然而,不同水平的呼气末正压或肺复张策略被纳入这些低潮气量通气策略中。除了低潮气量,很难确定哪些级别的呼气末正压和肺复张策略具有肺保护作用。我们将探索高呼气末正压和传统呼气末正压相比,能否有效改善机器人辅助肾部分切除术患者的术中氧和,降低术后肺部并发症,改善预后,减少住院天数。  

Objectives of Study:

Decades of research have attempted to determine the beneficial ventilation strategies by assessing tidal volume, positive end expiratory pressure, and alveolar recruitment maneuver.All of these studies, however, have produced mixed results.For tidal volume,there was conclusion that low tidal volumes ventilation could improve lung function better than high tidal volumes ventilation.However,different levels of positive end expiratory pressure or alveolar recruitment strategies were incorporated into these low tidal volume ventilatory strategies.With the exception of low tidal volume,it is difficult to determine which levels of positive end expiratory pressure and alveolar maneuver recruitment strategies confer lung protection.We will explore whether high positive end expiratory pressure can effectively improve intraoperative oxygenation, reduce postoperative pulmonary complications,improve prognosis,and reduce hospitalization days in patients undergoing Robot-assisted partial nephrectomy compared with traditional positive end expiratory pressure.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.ASAⅠ到Ⅱ级;
2.年龄大于18岁小于65岁,男女不限;
3.拟行机器人辅助肾部分切除术。

Inclusion criteria

1.American Society of Anesthesiology physical status Ⅰ or Ⅱ;
2.Patients aged 18 to 65 years;male or female;
3.Scheduled for Robot-assisted partial nephrectomy.

排除标准:

1.肺功能受损(慢性阻塞性肺疾病大于3级或1s内用力呼气量小于预测值30%);
2.慢性心力衰竭(心脏指数小于1.8 L/m2);
3.体重指数>40kg·m-2;
4.既往肺部手术史。

Exclusion criteria:

1.impaired lung function(chronic obstructive pulmonary disease grade greater than 3 or forced expiratory volume in 1s less than 1000ml)
2.chronic heart failure(cardiac index less than 1.8L/m2)
3.BMI>40kg·m-2
4.previous lung surgery
