研究目的：

探索不同主动脉瓣狭窄患者的升主动脉血流状态，以期能为主动脉瓣膜置换手术策略提供更多的动态血流信息，力争建立主动脉瓣膜置换术疗效评估模型，助力于提升主动脉瓣膜置换术的风险控制和疗效。  

Objectives of Study：

The aim of this study is to establish the efficacy evaluation model of aortic valve replacement to help improve the risk control, through evaluation the flow status of the ascending aorta in patients with different aortic stenosis before aortic valve replacement.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1) 心功能为NYHA （New York Heart Association）II级；
(2) 心脏超声提示严重主动脉瓣狭窄：主动脉瓣面积（aortic valve area，AVA ）< 1.0 cm2, 主动脉压力梯度（mean aortic pressure gradient，PGmean）> 40 mm Hg，峰值流速（peak systolic velocity， Vmax）> 4 m/s；
(3) 心脏冠脉CTA或者主动脉CTA确认存在瓣环/瓣周明显钙化的患者；升主动脉根部管径>40mm.

Inclusion criteria

1. symptomatic with New York heart association (NYHA) class >=II;
2. documented severe calcified native aortic stenosis by Echocardiography: aortic valve area (AVA) <1.0 cm2 (or AVA index <0.6 cm2/m2) , mean pressure gradient >=40 mmHg, or jet velocity >=4.0 m/s;
3. with significant annulus/perivalvular calcification confirmed by coronary CTA or aortic CTA; The diameter of ascending aortic root >40mm.

排除标准：

(1) 碘对比剂使用禁忌患者：曾有碘剂严重过敏病史，甲亢未能治愈，严重糖尿病肾病或肾功能严重受损（eGFR<30ml/min/1.73m2）；
(2) 意识不清，不能屏住气配合检查；
(3) 存在MR检查禁忌症患者（如：心脏起搏器植入、ICD植入，存在幽闭恐惧症患者）

Exclusion criteria：

(1) With contravariant use of iodine contrast agent: had a history of severe allergy to iodine agent, failed to cure hyperthyroidism, severe diabetic nephropathy or severely impaired renal function (eGFR<30ml/min/1.73m2);
(2) No conscious, can not hold breath to cooperate with the inspection;
(3) With contraindications to MR examination (such as pacemaker implantation, ICD implantation, and patients with claustrophobia).
