研究目的：

通过随机对照的临床研究，验证创新型复杂腹壁缺损多学科合作诊疗模式临床应用的有效性和安全性。  

Objectives of Study：

A randomized controlled clinical study was conducted to verify the effectiveness and safety of the innovative multi-disciplinary cooperative diagnosis and treatment model for complex abdominal wall defects.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1) 受试者本人或其法定代理人签署知情同意书。
2) 年龄18-75岁（包含18岁和75岁）,性别不限。
3) 符合“腹壁缺损修复与重建中国专家共识（2019 版）”复杂腹壁缺损诊断标准的患者：
a：腹壁缺损的大小与部位:巨大腹壁缺损（缺损宽度≥10cm）、腹壁缺损伴腹壁功能不全（loss of domain, LOD）、缺损发生在耻骨上和肋缘下等特殊部位的腹壁缺损。
b：腹壁软组织的情况:伴有污染或感染、腹壁肿瘤扩大切除术后、严重外伤、多次手术后、植皮后、伴溃疡或难愈创面及伴造口或瘘口旁的腹壁缺损。
c：病人全身情况与既往疾病史:伴有各种危险因素，如肥胖症、糖尿病、慢性阻塞性肺病、大量腹水、使用类固醇激素、营养状况差等疾病，有切口裂开病史、行补片或组织结构分离技术修补后复发的腹壁缺损。
d：临床状况:开放腹腔，或伴有肠管的嵌顿、绞窄须同时行肠切除，须同时行补片取出及多部位腹壁缺损。
4) 同意进行三维可视化与三维打印术前规划。

Inclusion criteria

1) The subject or his legal representative signed the informed consent form.
2) 18-75 years old (including 18 and 75 years old), regardless of gender.
3) Patients who meet the "Chinese Expert Consensus on Abdominal Wall Defect Repair and Reconstruction (2019 Edition)" diagnostic criteria for complex abdominal wall defects:
a: The size and location of the abdominal wall defect: huge abdominal wall defect (defect width ≥10cm), abdominal wall defect with abdominal wall dysfunction (loss of domain, LOD), and abdominal wall defect where the defect occurs in special parts such as the suprapubic and subcostal margins.
b: The condition of the soft tissue of the abdominal wall: with contamination or infection, after extensive resection of the abdominal wall tumor, severe trauma, after multiple operations, after skin grafting, with ulcers or hard-to-heal wounds, and with abdominal wall defects near the stoma or fistula.
c: The patient's general condition and history of past diseases: accompanied by various risk factors, such as obesity, diabetes, chronic obstructive pulmonary disease, large amounts of ascites, use of steroid hormones, poor nutritional status, etc., history of incision dehiscence, patching Or the recurrence of abdominal wall defect repaired by tissue structure separation technology.
d: Clinical conditions: Open abdominal cavity, or with incarceration or strangulation of intestinal tube, must undergo intestinal resection at the same time, patch removal and multiple abdominal wall defects must be performed at the same time.
4) Agree to plan for 3D visualization and 3D printing.

排除标准：

1) 本试验开始前 3个月内曾参加过其他临床试验的患者 ；
2) 妊娠、哺乳期女性及计划怀孕的患者；
3) 正在进行放射治疗或化学治疗的患者；
4) 心脑血管疾病未控制， NYHA 心功能分级 II 级或以上者
5) 经研究者判断患有严重内分泌代谢性疾病 、系统性免疫疾病 、血液 、呼吸系统疾病，不适合参加本次试验的患者；
6) 全身有未控制的感染者；
9) 肝疾病 或肝损伤的患者（ ALT和AST 超过正常值上限的2倍）；
10) 严重肾功能 损伤或肾衰竭的患者 （血肌酐 大于正常值上限 的2 倍）；
11) 无法定期完成随访的患者。
12) 酗酒、吸毒或其他药物成瘾者有倾向；
13) 不能耐受增强CT检查的患者；
14) HIV抗体初筛检测阳性者；

Exclusion criteria：

1) Patients who have participated in other clinical trials within 3 months before the start of this trial;
2) Pregnant, lactating women and patients planning to become pregnant;
3) Patients undergoing radiotherapy or chemotherapy;
4) Those with uncontrolled cardiovascular and cerebrovascular diseases and NYHA heart function classification II or above
5) Patients who have been judged by the researcher to have serious endocrine and metabolic diseases, systemic immune diseases, blood, and respiratory diseases, are not suitable for participating in this trial;
6) There are uncontrolled infections throughout the body;
9) Patients with liver disease or liver injury (ALT and AST exceeding 2 times the upper limit of normal);
10) Patients with severe renal impairment or renal failure (serum creatinine is greater than 2 times the upper limit of normal);
11) Patients who cannot complete follow-up regularly.
12) People who are addicted to alcohol, drugs or other drugs have a tendency;
13) Patients who cannot tolerate enhanced CT examination;
14) Those who are positive in the initial HIV antibody screening test;
