研究目的：

利用社区服务中心的全科门诊，对糖尿病患者提供个体化的健康教育，提高社区的糖尿病规范管理率和控制率，改善患者生活质量，促进分级诊疗。  

Objectives of Study：

To provide personalized health education for diabetes patients through the general clinic of the community service center and improve the standardized management rate and control rate of diabetes in the community, improve the quality of life of patients, promote graded diagnosis and treatment.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1 确诊为2型糖尿病患者，入组前3个月内HbA1C≥7.5%；
2 本试验开始前6月未参加过其他生活方式研究和糖尿病课题者；
3 受试者神智精神正常，自愿参加研究并签署知情同意书；
4 年龄45-70岁之间。
5 用药依从性≤6分

Inclusion criteria

1. Patients diagnosed with type 2 diabetes, HbA1C>=7.5% within 3 months before enrollment;
2. Had not participated in other lifestyle studies and diabetes subjects in 6 months before the beginning of this study;
3. Mentally and mentally normal, voluntarily participate in the study and sign the informed consent;
4. Aged 45 to 70 years old;
5. The medication compliance score <=6 points.

排除标准：

1 对本项研究不理解、不配合按时随访者；
2 有精神疾患、不能很好合作者；
3 恶性肿瘤及血液病患者；
4 肢体功能障碍影响行为干预活动实施者；
5 本试验开始前6月参加过其他生活方式研究和糖尿病课题者；
6 患糖尿病严重并发症者（严重糖尿病周围血管病变、严重糖尿病周围神经病变、糖尿病肾病终末期等）

Exclusion criteria：

1. Do not understand this study, can not accept regular follow-up;
2. With mental disorders,can not cooperate well;
3. With malignant tumors and blood diseases;
4. Physical dysfunction affects the implementer of behavioral intervention activities;
5. Participants who had participated in other lifestyle studies and diabetes subjects 6 months before the beginning of this experiment;
6. Patients with serious complications of diabetes (severe diabetic peripheral vascular disease, severe diabetic peripheral neuropathy, diabetic end-stage renal disease, etc.)

采用二阶段整群随机抽样方法，研究人员首先从高碑店社区卫生服务中心8个站点、朝阳区双井社区卫生服务中心6个站点及三里屯社区卫生服务中心6个站点共20站点中，随机抽取3个站点为干预组，3个站点为对照组，每个站点对管理的糖尿病患者再进行随机抽样，每个站点纳入30人，每组90人，共180人。
