利用中枢-外周神经环路磁调控技术,创新性地早期激活脊髓丘脑束-感觉运动皮层-皮质脊髓束环路的活性,促进损伤半暗带区的神经环路纤维修复与新网络重建,结合任务导向的协同运动功能训练,有效提高感觉运动神经调控的效率,并建立损伤区神经环路重建的电生理和影像大数据。  

Objectives of Study:

Using central - peripheral neural circuits of magnetic control technology, innovative and early activation of spinal cord thalamus - sensorimotor cortex - the activity of the corticospinal tract loop, promote restoration of the neural circuits fiber damage half dark band area with the new network reconstruction, combined with task-oriented collaborative movement function training, effectively improve the efficiency of sensorimotor neural control, neural circuits and establish the damaged area large data of electrophysiology and image reconstruction.

药物成份或治疗方案详述:

基于中枢-外周神经环路磁调控技术在缺血性脑卒中早期的康复疗效研究,验证该康复方法对运动功能障碍患者功能恢复的安全性和有效性,同时进行脑重塑机制研究,探索推拿及环路神经磁调控促进功能恢复的脑重塑机制,并制定相应的临床应用规范和标准。具体而言:筛选入组运动功能障碍的缺血性脑卒中患者280例,伪刺激+常规康复组和磁刺激+常规康复组。分别在治疗前、治疗开始后第1月、2月、3月、6 月内对其运动功能进行定量评估和综合疗效分析,组间进行对比分析,对中枢-外周神经环路磁调控干预在缺血性卒中早期(2天-1月)上肢运动功能恢复方面的效果进行安全性和有效性验证,并通过功能磁共振成像(fMRI)、电生理等方法检测,探索环路神经调控及推拿促进功能恢复的脑重塑机制,并制定环路神经调控协同推拿康复训练技术应用规范和标准。 磁刺激+常规康复组的患者在常规康复治疗的基础上使用重复经颅磁刺激仪(上海依瑞德公司),磁刺激线圈为“8”字型。首先确定静息运动阈值(resting motor threshold, RMT):将表面记录电极放置于刺激靶点健侧拇短展肌肌腹处,参考电极置于肌腱处,地线接腕部,调整磁刺激线圈中心点位置至最佳诱发患者健侧大脑皮层拇短展肌运动诱发电位(motor evoked potential, MEP)位置,然后逐渐减小刺激量,在静息状态下,10次刺激中有至少 5次刺激下 MEP 波幅 > 50μV的最小磁刺激强度作为该侧大脑皮质RMT。治疗时患者仰卧位,以最佳诱发患者MEP的位置作为刺激靶点。“8”字形线圈与颅骨表面相切,中点对准刺激靶点,刺激强度为100% RMT,治疗频率为 1Hz,每次治疗总脉冲数为1200个,持续时间20 min,每日1次,每周5次,共4周。 伪刺激+常规康复组治疗时患者取仰卧位,刺激部位、频率、刺激时长、治疗周期均与观察组相同,治疗时将刺激线圈旋90°,使线圈与颅骨表面垂直。 外周磁刺激方案:磁刺激组患者取半仰卧位,以患侧上肢Erb氏点(锁骨上窝内下角,即锁骨上2~3cm约在胸乳肌后一横指处)作为刺激靶点。“8”字形线圈切面贴近胸壁,中点对准刺激靶点,刺激强度为100% RMT,治疗频率为 5Hz,次间时常每次治疗总脉冲数为1200个,每日1次,每周5次,共4周。 伪刺激组患者取半仰卧位,刺激部位、频率、刺激时长、治疗周期均与相同,治疗时将刺激线圈旋90°,使线圈与胸壁垂直。 所有入组患者均予以常规康复治疗。 

Description for medicine or protocol of treatment in detail:

Based on central - peripheral neural circuits of magnetic control technology in early ischemic stroke rehabilitation efficacy study, verify that the methods of rehabilitation on motor function disorder in patients with functional recovery of the safety and effectiveness, and reshape the brain mechanism research, explore the massage and promote the functional recovery of brain restore nerve magnetic loop regulation mechanism, and formulate the corresponding clinical application of norms and standards. To be specific, 280 patients with ischemic stroke with motor dysfunction were selected, and the pseudo-stimulation + conventional rehabilitation group and the magnetic stimulation + conventional rehabilitation group were selected. Respectively before treatment, treatment after the start of January, February, march, the first 6 months of its movement function for quantitative evaluation and comprehensive curative effect analysis, comparison analysis between groups, on the central - peripheral neural circuits of magnetic control intervention in the early ischemic stroke (2 days) - 1 month of upper limb movement function recovery effect verifies the safety and effectiveness, and through functional magnetic resonance imaging (fMRI) and electrophysiological methods, explore the neural regulation loop and reshape the brain mechanism of massage to promote functional recovery, and make the loop neural regulation synergy massage and rehabilitation training technology norms and standards. Patients in the magnetic stimulation + routine rehabilitation group were treated with a repetitive transcranial magnetic stimulator (Shanghai Reed) on the basis of routine rehabilitation treatment. The magnetic stimulation coil was shaped like "8". First, determine the RESTING motor threshold (RMT) : Record surface electrodes placed in thumb short abductor muscle stimulation target the contralateral muscle belly, reference electrode placed in the tendon, ground wire to the wrist, adjust the magnetic stimulation coil center location to best patients induced the contralateral thumb short abductor muscle movement cerebral cortex evoked potential (motor evoked potential, MEP) position, and then gradually reduce the amount of stimulus, in the resting state, 10 times under the stimulus of at least five times the minimum magnetic stimulation MEP amplitude > 50 mu V intensity as the lateral cortex RMT. During the treatment, the patient was in the supine position, and the best position to induce MEP was used as the stimulation target. The "8" shape coil was tangent to the skull surface, the midpoint was aligned to the stimulation target, the stimulation intensity was 100% RMT, the treatment frequency was 1Hz, the total pulse number of each treatment was 1200, the duration was 20 min, once a day, 5 times a week, for 4 weeks. Patients in the pseudo-stimulation + conventional rehabilitation group were placed in the supine position during treatment, and the stimulation site, frequency, duration and treatment cycle were the same as those in the observation group. During treatment, the stimulation coil was rotated 90° to make the coil perpendicular to the skull surface. Peripheral magnetic stimulation: The patients in the magnetic stimulation group were placed in the semi-supine position, and the Erb point of the affected upper limb (the inferior Angle of the supraclavicular fossa, i.e., the supraclavicular supraclavicular 2 ~ 3cm at the posterior horizontal finger of the pectoralis muscle) was taken as the stimulation target. The cut surface of the "8" shape coil was close to the chest wall, the midpoint was aligned with the stimulus target, the stimulus intensity was 100% RMT, the treatment frequency was 5Hz, and the total pulse number of each treatment was often 1200, once a day, 5 times a week, for 4 weeks. Patients in the pseudo-stimulation group were placed in the semi-supine position, with the same stimulation site, frequency, duration and treatment cycle. During the treatment, the stimulation coil was rotated 90° to make the coil perpendicular to the chest wall. All enrolled patients received routine rehabilitation treatment. 

纳入标准:

① 临床评估和综合影像学检查确诊为缺血性脑卒中的患者,符合缺血性脑卒中诊断标准;
② Fugl-Meyer运动功能评分证实为运动功能障碍;
③ 年龄18-70周岁,男女不限;
④ 无认知障碍或交流障碍,简易智能状态检查(MMSE)27分以上;
⑤ 首次发病,病程2天-1月。

Inclusion criteria

1. Patients with ischemic stroke confirmed by clinical assessment and comprehensive imaging examination meet the diagnostic criteria for ischemic stroke;
2. Fugl-meyer score of motor function was confirmed as motor dysfunction;
3. Aged 18-70 years, men and women are not limited;
4. No cognitive impairment or communication impairment, MMSE score above 27;
5. The first onset was 2 days to 1 month.

排除标准:

① 有心肺疾病等严重系统性疾病不能耐受康复治疗者;
② 确诊精神病、重度抑郁症(有自杀倾向)或癫痫患者、有精神病或癫痫的家族史者;
③ 存在糖尿病、尿毒症等严重系统性疾病;
④ 严重的关节挛缩;
⑤ 患有任何原因导致意识障碍;
⑥ 根据安全指南有经颅直流电/磁刺激(TMS/tDCS)和功能磁共振(fMRI)检查的禁忌证如:体内有金属异物或有其它植入体内电子装置等;
⑦ 听觉或视觉上有缺陷可能影响评估及治疗;
⑧ 应用改变大脑皮质兴奋性的药物(抗癫痫药、镇静催眠类药物等)。

Exclusion criteria:

1. Patients with severe systemic diseases such as cardiopulmonary diseases and unable to tolerate rehabilitation treatment;
2. Diagnosed with mental illness, major depression (suicidal tendency) or epilepsy, family history of mental illness or epilepsy;
3. The presence of diabetes, uremia and other serious systemic diseases;
4. severe joint contracture;
5. Suffer from any reason leading to consciousness disorder;
6. According to the safety guidelines there are transcranial direct current/magnetic stimulation (TMS/tDCS) and functional magnetic resonance imaging (fMRI) check contraband such as: there is a metal foreign body or other electronic devices implanted in the body;
7. Auditory or visual impairments that may affect assessment and treatment;
8. Apply the drug that changes cerebral cortex excitability today (antiepileptic drug, sedative hypnotic kind medicaments).
