研究目的：

针对CT影像中含有GGO成分的Ia期肺腺癌患者，比较微创肺叶切除术或肺段切除术对患者预后及围术期风险，肺功能影响。主要观察终点：患者预后。次要观察终点：患者围手术期并发症和肺功能的影响。  

Objectives of Study：

The treatment effects of minimally invasive of lobectomy or segmentectomy will be compared on the prognosis, perioperative complications, and pulmonary function in patients of stage Ia lung adenocarcinoma with GGO components in CT scans. Primary end point: patient outcomes. Secondary endpoints: perioperative complications and lung function of patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄：20-80岁；
2.临床怀疑为非小细胞肺癌，CT表现位于肺野外周1/3，含有GGO成分的混合性结节；
3.临床分期（第八版UICC分期）Ia期（T1a-cN0M0），病灶总体最大直径≤3cm，实密成分最大径 >0.5cm且≤2cm；
4.多发GGO考虑为多原发肺癌、但不存在最大直径>1cm、实密成分 >0.5cm、可能在未来5年内需要手术治疗的第二主病灶者仍可纳入；
5.未经任何诱导治疗；
6.PS评分0-1，无严重合并症，功能状况可耐受微创肺叶切除。

Inclusion criteria

1.Age at 18-75 years;
2.The mixed nodules containing GGO components were found in outside 1/3 of the lung field by CT scans, and were clinically suspected to be non-small cell lung cancer;
3.Clinical staging (UICC eighth edition) stage Ia (T1a-cN0M0), overall maximum diameter of lesion ≤3cm, maximum diameter of solid density component>0.5 cm and ≤2 cm or less
4.Multiple GGOs were considered as multiple primary lung cancer, but there are no maximum diameter>1cm, solid dense composition>0.5cm, and the second major lesion that may require surgical treatment in the next 5 years
5.The patient did not receive any induction therapy;
6.The patient had PS score of 0-1, no serious complications, and was able to tolerate minimally invasive lobectomy.

排除标准：

1.CT表现不含有GGO成分的纯实密结节、或实密成分≤0.5cm的纯GGO；
2.病灶位于肺野内、中1/3，肺段切除可能无法满足切缘要求；
3.多发GGO考虑有多原发肺癌、且除主病灶外存在最大直径≥1cm、实密成分≥0.5cm、可能在未来5年内需要手术治疗的第二主病灶；
4.合并严重其它脏器疾病，手术治疗高风险患者；
5.既往5年内有其它恶性肿瘤病史；
6.既往有同侧胸部手术病史。

Exclusion criteria：

1. Pure solid nodules without GGO components, or pure GGO with solid dense components ≤0.5cm in CT scans
2. The lesions were located in the lung field of inside 1/3, and segmentectomy may not meet the requirements of the cutting edge;
3. Multiple GGOs were considered as multiple primary lung cancers. In addition to the main lesion, the second main lesion with the largest diameter ≥1cm and dense component ≥0.5cm, which may require surgical treatment in the next 5 years;
4. Patients with severe comorbidities and a high risk of surgery;
5. History of other malignant tumors in the past 5 years;
6. A history of ipsilateral thoracic surgery.
