研究目的：

（1）确认多剂量亚甲蓝预防老年结直肠癌根治术患者术后短期和长期神经认知紊乱的有效性； （2）寻找亚甲蓝预防老年患者术后认知功能障碍的血浆生物标志物和神经影像标志物，从而为新药或新方法改善术后认知损害提供有价值的评价指标； （3）观察亚甲蓝对围术期肿瘤相关标志物的影响，以及术后2年无病生存期（DFS），无进展生存期（PFS） 以及总体生存率（OS）。  

Objectives of Study：

(1) To confirm the effectiveness of multi-dose methylene blue in preventing short-term and long-term neurocognitive disorders after radical resection of colorectal cancer in elderly patients; (2) To find plasma biomarkers and neuroimaging markers for methylene blue to prevent postoperative cognitive dysfunction in elderly patients, so as to provide valuable evaluation indicators for new drugs or new methods to improve postoperative cognitive impairment; (3) To observe the effect of methylene blue on perioperative tumor-related markers, as well as disease-free survival (DFS), progression-free survival (PFS) and overall survival (OS) at 2 years after surgery.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①年龄60-80岁；
②拟行结直肠癌根治手术的老年患者；
③MMSE ≥ 24；
④愿意参加整个研究过程。

Inclusion criteria

① aged 60-80 years old
②planning to undergo colorectal cancer resection
③MMSE ≥ 24
④ Patients have the ability to act in full spirit, understand and sign the
informed consent, and are willing to complete the whole research process.

排除标准：

①亚甲蓝使用禁忌症患者：如对亚甲蓝过敏或6-磷酸-葡萄糖脱氢酶缺乏症（蚕豆病）；
②近期有选择性5 -羟色胺再摄取抑制剂和5-羟色胺-去甲肾上腺素再摄取抑制剂服药史；
③有重要精神或神经疾病或毒品或酒精滥用史；
④有严重内科疾病，如心力衰竭、呼吸衰竭或肝肾功能障碍等；
⑤重大头颅外伤史或颅内肿瘤及手术史；
⑥有严重语言、听觉、视觉障碍而无法交流的患者；
⑦近3月参加其它临床实验者

Exclusion criteria：

①preexisted dementia, major depression or other serious mental or neurological disorders
② history of allergy to MB or 6-phospho-glucose dehydrogenase deficiency (favism)
③drug or alcohol abuse or recent drug administration that may lead to drug interactions, such as selective serotonin reuptake inhibitors (SSRIs) or serotonin-norepinephrine reuptake inhibitors (SNRIs)
④history of major head trauma
⑤serious medical diseases (ie. heart failure, pulmonary hypertension,
⑥acute stage of myocardial infarction or respiratory failure, liver and kidney dysfunctions)
severe language, visual or auditory deficiency
⑦participated in other clinical trials within 3 months.

麻醉诱导后静脉泵注2mg/kg的亚甲蓝稀释液共50ml（0.9%生理盐水稀释），持续时间1小时；手术结束前30分钟，静脉泵注1mg/kg的亚甲蓝稀释液共50ml（0.9%生理盐水稀释）至手术结束。

本试验采用复旦大学生物医学统计中心中央随机化系统完成入组病例的随机分组。本研究在随机化分组过程中的主要分层因素包括：术前合并疾病和术前认知评分。 本试验采用复旦大学公共卫生学院生物医学统计中心临床研究数据采集与管理系统（Electronic Data Capture，EDC）完成多中心临床试验数据的收集。该系统涵盖临床研究的整个过程, 实现数据与过程的可溯源性。
