研究目的：

后纵韧带骨化（Ossification of the posterior longitudinal ligament， OPLL）是脊柱外科一种常见的疾病。附着于椎体后缘的后纵韧带在多因素作用下形成骨化物，骨化物逐渐生长可压迫脊髓和神经根造成一系列压迫症状，包括四肢感觉、运动、反射以及二便功能障碍的综合征，严重降低患者生活质量，给家庭及社会带来沉重的负担。尤其当OPLL发生在颈椎时，造成的脊髓及神经损伤更为严重，发病率、致残率、致伤率更高。本项目拟通过对患者骨化组织样本测序及免疫组化染色研究OPLL的发病机制。  

Objectives of Study：

Ossification of the posterior longitudinal ligament (OPLL) is a common disease in spinal surgery. The posterior longitudinal ligament attached to the posterior edge of the vertebral body forms ossification under the action of multiple factors. The gradual growth of the ossification can compress the spinal cord and nerve roots and cause a series of compression symptoms, including syndromes of limb sensation, movement, reflex, and fecal dysfunction. , Which seriously reduces the quality of life of patients, and brings a heavy burden to the family and society. Especially when OPLL occurs in the cervical spine, the spinal cord and nerve damage caused is more serious, and the incidence, disability, and injury rate are higher.This project intends to study the pathogenesis of OPLL through sequencing and immunohistochemical staining of patient ossified tissue samples.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

入选颈椎后纵韧带骨化症组标准：
①经CT三维重建证实的颈椎后纵韧带单节段或多节段骨化，或至少同时合并后纵韧带骨化、黄韧带骨化、弥漫性骨质增生中两种疾病的；
②年龄18岁至75岁之间，性别不限；
③造成脊髓或神经根压迫症状，有手术指征且患者手术意愿强烈；
④愿意签署知情同意书。
入选颈椎后纵韧带无骨化症组标准：
①脊柱无后纵韧带骨化；仅有椎间盘退变突出；
②年龄18岁至75岁之间，性别不限；
③造成脊髓或神经根压迫症状，有手术指征且患者手术意愿强烈；
④愿意签署知情同意书。

Inclusion criteria

Criteria for inclusion in the cervical posterior longitudinal ligament ossification group:
1. Single-segment or multi-segment ossification of the posterior longitudinal ligament of the cervical spine confirmed by CT three-dimensional reconstruction, or at least combined with two diseases of posterior longitudinal ligament ossification, ligamentum flavum ossification, and diffuse bone hyperplasia;
2. Aged Between 18 and 75 years old;
3. Symptoms of spinal cord or nerve root compression, surgical indications and the patient's willingness to operate;
4. Willing to sign informed consent.
Criteria for inclusion in the cervical posterior longitudinal ligament without ossification group:
1. No posterior longitudinal ligament ossification of the spine; only degeneration and herniated intervertebral disc;
2. Aged Between 18 and 75 years old;
3. Symptoms of spinal cord or nerve root compression, surgical indications and the patient's willingness to operate;
4. Willing to sign informed consent.

排除标准：

①患有未控制的高血压、糖尿病或其它严重的心、肺疾病等手术禁忌症患者；
②凝血功能极差，血小板低于20×109/L者或INR≥3.5者；
③确诊或怀疑合并恶性肿瘤者；
④妊娠哺乳期妇女或不排除妊娠可能的妇女；
⑤患有精神类疾病者；
⑥其它研究者认为不适合参加者。

Exclusion criteria：

1. Patients with uncontrolled hypertension, diabetes or other severe heart and lung diseases and other surgical contraindications;
2. Very poor coagulation function, platelets lower than 20x10^9/L or INR>=3.5;
3. Patients diagnosed or suspected of having malignant tumor;
4. Pregnant or lactating women, or women whose pregnancy is unclear;
5. Patients suffering from mental illness;
6. Other researchers think it is not suitable for participants.
