研究目的：

卵巢癌患者腹腔中潜在的转移的肿瘤细胞目前并无有效的应对措施，腹腔热灌注化疗（HIPEC）是将能够直接作用于腹腔内残留肿瘤细胞的腹腔化疗和具有多重抗肿瘤作用的热效应两者相结合的一种治疗腹腔恶性肿瘤的辅助方法。尽管HIPEC对于治疗卵巢癌具有显著优势，但是目前支持将HIPEC应用于新发卵巢癌临床治疗的高质量证据并不充分。为推动我国HIPEC治疗卵巢癌指南的制订，提高我国女性卵巢恶性肿瘤的生存率，本项目旨在研究国内在新发诊断卵巢癌中相比于常规手术加全身化疗，通过联合手术和术中HIPEC及全身化疗对患者生存预后的影响。  

Objectives of Study：

There are currently no effective measures against potential metastatic tumor cells in the abdominal cavity of patients with ovarian cancer. As a treatment of abdominal malignant tumors, hyperthermic intraperitoneal chemotherapy (HIPEC) is a combination of intraperitoneal chemotherapy that can directly act on residual tumor cells in the abdominal cavity and thermal effects with multiple anti-tumor effects. Although HIPEC has significant advantages in the treatment of ovarian cancer, the current high-quality evidence supporting the application of HIPEC in the clinical treatment of new diagnosed ovarian cancer is insufficient. In order to promote the formulation of guidelines for the treatment of ovarian cancer by HIPEC and improve the survival rate of women with ovarian cancer in China, this project aims to study the impact of the combination of surgery and intraoperative HIPEC in the newly diagnosed ovarian cancer on the survival and prognosis of patients compared with conventional surgery and systemic chemotherapy.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄≥18岁；
（2）浆液性卵巢癌、粘液性卵巢癌、子宫内膜样卵巢癌、输卵管癌或原发腹膜癌；
（3）新诊断的FIGO Ⅲ期（包括接受过新辅助化疗）；
（4）手术达到完全满意灭减或残余病灶<1cm；
（5）无其他腹腔脏器疾病及内脏转移；
（6）体力状况ECOG评分为0或1分；
（7）血常规、凝血功能及心肝肾功能正常。

Inclusion criteria

(1) Age ≥ 18 years old;
(2) Serous ovarian cancer, mucinous ovarian cancer, endometrioid ovarian cancer, fallopian tube cancer or primary peritoneal cancer;
(3) Newly diagnosed FIGO Phase III (including having received neoadjuvant chemotherapy);
(4) The operation achieves complete reduction or residual disease less than 1cm;
(5) No other abdominal organ diseases and visceral metastasis;
(6) The ECOG score of physical strength is 0 or 1;
(7) Blood routine, coagulation function and heart, liver and kidney function are normal.

排除标准：

（1）存在胸膜疾病或肺转移证据；
（2）腹膜、肠系膜肿胀或肠梗阻；
（3）入组前5年内患有乳腺癌或其他腹盆腔恶性肿瘤疾病；
（4）可能存在术后吻合口愈合不良高危因素，包括吻合口组织缺血水肿、张力明显、严重低蛋白血症等。

Exclusion criteria：

(1) There is evidence of pleural disease or lung metastasis;
(2) Peritoneum and mesenteric swelling or intestinal obstruction;
(3) Suffered from breast cancer or other abdominal and pelvic malignancies within 5 years before enrollment;
(4) There may be high-risk factors for poor anastomotic healing after operation, including ischemic edema of anastomotic tissue, obvious tension, and severe hypoproteinemia.
