研究目的：

本研究拟设计一项前瞻性的临床对照试验，采用国际化标准评价方法，使用平衡训练系统联合数字化跑台的训练模式,并加用重复经颅磁刺激，对帕金森病及帕金森综合证患者的平衡及步态进行康复干预,采用三维步态分析系统检测，同步观测和分析患者下肢步态的动态三维运动学（步长、步速、步态周期、关节活动度和骨盆旋转）变化，使用国际通用的 MMSE 量表评分及蒙特利尔 MoCA 认知评估量表对帕金森病及帕金森综合证患者进行认知评估，并采用帕金森评分量表（UPDRS）、Berg 量表、Tinetti步态评分量表、Barthel 指数量表、Morse 跌到评估量表，进行干预前后患者平衡、步态、日常生活能力及跌倒风险的评估。  

Objectives of Study：

In this study, a prospective controlled clinical trial was designed. Using the international standard evaluation method, the balance training system combined with digital treadmill training mode was used, and repetitive transcranial magnetic stimulation was used to rehabilitate the balance and gait of patients with Parkinson's disease and Parkinson's syndrome, Three dimensional gait analysis system was used to detect and analyze the dynamic three-dimensional kinematics (step length, gait speed, gait period, joint range of motion and pelvic rotation) changes of the lower limb gait of the patients. The international MMSE scale score and Montreal MOCA were used Patients with Parkinson's disease and Parkinson's syndrome were assessed with cognitive assessment scale. Parkinson's scale (UPDRS), Berg scale, Tinetti gait scale, Barthel index scale and Morse scale were used to evaluate the balance, gait, activities of daily living and fall risk before and after the intervention.

纳入标准：

1.帕金森病组（PD 组）：①起病缓 慢，病程≥3 年；②静止性震颤、运动障碍或强直至少出现 2 项；③左旋多巴治疗有反应。 ④患者血压平稳（100-160/60-100mmHg）；⑤同意进行系统的认知和运动功能评价，能够 理解治疗师或仪器指令，并能完成所要求的动作。⑥通过甘肃省人民医院伦理委员会批准。同意参加本研究，签署知情同意书。
2.帕金森综合征组（PS 组）：①有帕金森综合征的表现，即必须具有运动迟 缓，并具有下列症状之一：静止性震颤、肌强直 和姿势不稳，姿势不稳排除由原发性视觉、 前庭、小脑及本体感觉异常引起。②具有脑血管病的表现，可以为脑影像学的表现，也可以 是由卒中引起的局灶性症状和体征。③上述①和②之间必须有关联：卒中后急性发病或在 1 年内逐渐出现帕金森综合征的表现，卒中受累部位主要引起基底节区运动输出功能增强 （苍白球外侧部或黑质致密部）或丘脑皮质通路 功能减低（丘脑的腹后外侧核，额叶大面 积梗死），导致对侧肢体以少动-强直为主要表现的帕金森综合征；隐匿性起病、由皮质下脑 白质损害引起的早期双下肢步态障碍或认知功能障碍。④患者血压平稳（100-160/60100mmHg）；⑤同意进行系统的认知和运动功能评价，能够理解治疗师或仪器指令，并能 完成所要求的动作。⑥通过甘肃省人民医院伦理委员会批准。同意参加本研究，签署知情同意书。

Inclusion criteria

1. Parkinson's disease group (PD group): ① the onset is slow, the course of disease is more than 3 years; ② there are at least 2 cases of static tremor, dyskinesia or ankylosis; ③ levodopa treatment has response. ④ The patients' blood pressure was stable (100-160 / 60-100mmhg); ⑤ they agreed to carry out systematic cognitive and motor function evaluation, and could understand the instructions of therapists or instruments and complete the required actions. ⑥ It was approved by the ethics committee of Gansu Provincial People's hospital. They agreed to participate in the study and signed the informed consent.
2. Parkinson's syndrome group (PS group): ① it has the symptoms of Parkinson's syndrome, that is, it must have the movement retardation, and has one of the following symptoms: static tremor, myotonia and postural instability. Postural instability is excluded from primary visual, vestibular, cerebellar and proprioception abnormalities. ② With the performance of cerebrovascular disease, can be the performance of brain imaging, can also be caused by stroke focal symptoms and signs. ③ There must be a correlation between the motor function of the ventral thalamus and the lateral part of the thalamus The results showed that the main manifestation of the contralateral limb was Parkinson's syndrome with less movement and rigidity; latent onset, early gait disorder or cognitive dysfunction caused by subcortical white matter damage. ④ The patients' blood pressure was stable (100-160 / 60100 mmHg); ⑤ they agreed to carry out systematic cognitive and motor function evaluation, and could understand the instructions of therapists or instruments and complete the required actions. ⑥ It was approved by the ethics committee of Gansu Provincial People's hospital. They agreed to participate in the study and signed the informed consent.

排除标准：

1.帕金森病组（PD 组）：①帕金森叠加综合征及帕金森综合征者；② 体内放置金属内置物者；③有 癫痫病史者；④其他严重躯体性疾病或精神疾病者。

2.帕金森综合征组（PS 组）：①反复颅脑外伤；确诊脑炎；②起病时有抗精神病药物治疗史；③ MRI 或 CT 证实 脑肿瘤或交通性脑积水；④其他原因引起的帕金森综合征等。⑤帕金森病患者；⑥体内放置金属内置物者；⑦有癫痫病史者；⑧其他严重躯体性疾病或精神疾病者。

Exclusion criteria：

1. Parkinson's disease group (PD group): ① patients with Parkinson's syndrome and Parkinson's syndrome; ② patients with metal implants in the body; ③ patients with epilepsy history; ④ patients with other serious physical or mental diseases.
2. Parkinson's syndrome group (PS group): ① repeated traumatic brain injury; confirmed encephalitis; ② history of antipsychotic drug treatment at the onset; ③ brain tumor or communicating hydrocephalus confirmed by MRI or CT; ④ Parkinson's syndrome caused by other reasons. ⑤ (6) patients with severe epilepsy or other mental diseases.
