研究目的：

主要目的：比较术中冰冻病理评估髂总淋巴结有癌转移后，进一步行腹主动脉旁淋巴结清扫术（肠系膜下动脉水平），与IB1期以上直接行腹主动脉旁淋巴结清扫术，肿瘤治疗结局，包括总生存率及无复发生存率。 次要目的：观察术中髂总淋巴结冰冻病理评估组与IB1期以上直接行腹主动脉旁淋巴结清扫术，术后髂总淋巴结阳性率、扩大放疗野范围，放疗相关副反应及生活质量。  

Objectives of Study：

Main objective: To compare the prognosis of para-aortic lymphadenectomy when frozen section analysis of common iliac lymph node was positive in cervical cancer and randomized radical hysterectomy with or without para-aortic lymphadenectomy in cervical cancer patient, including overall survival rate and recurrence-free survival rate. Secondary objective: To observe positive rate of common iliac lymph nodes, expansion of radiotherapy field, radiotherapy-related side effects and quality of life.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1)临床诊断为宫颈癌（IB1-IIA2）；
2)术前病理诊断为宫颈鳞癌、宫颈腺鳞癌或宫颈腺癌
3)年龄≤65岁；
4)初次治疗者；
5)受试者自愿加入本研究，签署知情同意书，依从性好，配合随访；

Inclusion criteria

1) Clinical diagnosis of cervical cancer (IB1-IIA2);
2) Preoperative pathological diagnosis of cervical squamous cell carcinoma, cervical adenosquamous carcinoma or cervical adenocarcinoma
3) Age ≤65 years;
4) Initial treatment;
5) Subjects Volunteer to join the study, sign an informed consent form, have good compliance, and cooperate with follow-up;

排除标准：

1)一般情况差，严重内科合并症难以耐受手术治疗者；
2)既往接受盆腔/腹部放疗照射或化疗的患者；
3)妊娠期患者
4)合并其他妇科恶性疾病如卵巢肿瘤的患者；
5)CT、MRI或PET提示子宫颈外转移；
6)不能或不愿意签署知情同意书者，不能或不愿意遵守研究要求者；

Exclusion criteria：

1)Patients' general condition is poor and severe medical complications are difficult to tolerate surgical treatment;
2) Patients who have previously received pelvic/abdominal radiotherapy or chemotherapy;
3) Patients during pregnancy
4) Patients with other gynecological malignancies such as ovarian tumors;
5) CT, MRI or PET suggesting extracervix metastasis;
6) Those who are unable or unwilling to sign the informed consent form, and are unable or unwilling to comply with the research requirements;
