研究目的:

评价mNGS在肺肉芽肿性病变活检组织学标本直接检出病原菌的效率,评价mNGS对肺部肉芽肿性疾病多学科诊断的增益。  

Objectives of Study:

To evaluate the efficiency of mNGS in the direct detection of pathogenic bacteria in biopsy histological specimens of pulmonary granulomatous lesions, and to evaluate the gain of mNGS in the multidisciplinary diagnosis of pulmonary granulomatous diseases.

纳入标准:

1)本研究仅限于肺活检和手术标本,包括经支气管镜活检,CT或者超声引导下活检、手术活检(肺楔活检/肺叶切除术,开胸或胸腔镜辅助)。
2)由病理科医生2名,复查玻璃片以确认肉芽肿性炎症的存在。
3)记录纳入患者的临床信息作为基线信息,包括年龄、性别、患者的免疫状态、体温、全血细胞计数、血沉、C反应蛋白、痰涂片、微生物免疫学和血清学检测,以及是否进行了经验性的抗生素治疗。

Inclusion criteria

1. This study is limited to lung biopsy and surgical specimens, including transbronchial biopsy, CT or ultrasound-guided biopsy and surgical biopsy (lung wedge biopsy / lobectomy, thoracotomy or thoracoscopic assistance).
2. The presence of granulomatous inflammation is confirmed by 2 pathologists.
3. The clinical information of the included patients is recorded as baseline information, including age, gender, immune status, body temperature, whole blood cell count, ESR, C-reactive protein, sputum smear, microbial immunology and serological tests, and whether empirical antibiotic therapy was performed.

排除标准:

1)排除因组织标本质量不合格,无法进行所有研究所需检测者。
2)排除因各种原因在治疗随访过程中中止随访或者失访者。
3)排除合并严重心肺全身疾病者、HIV阳性者。

Exclusion criteria:

1. Patients who are unable to carry out all the tests required for the study due to unqualified tissue samples.
2. Patients who have stopped or lost follow-up due to various reasons.
3. Patients with severe cardiopulmonary diseases and HIV positive patients.

金标准或参考标准(即可准确诊断某疾病的单项方法或多项联合方法,在本研究中用于诊断是否有该病的临床参考标准):

感染性肉芽肿的临床诊断被分为“确诊”和“可能”:被列为“确诊”的感染实体,在组织病理、病原学(包括mNGS)、血清学或抗原检测中均为一致的阳性表现,并且针对性的抗生素治疗有效。“可能的”感染诊断基于临床怀疑,血清学或抗原检测或既往感染史和相应的组织病理学,并且针对性的抗生素治疗有效。由于分枝杆菌不能在组织学标本中清楚区分,因此结核分枝杆菌与非结核分枝杆菌的确定是基于分枝杆菌培养的。在病理标本中发现分枝杆菌,但未进行培养、不能培养或阴性的情况下,该生物被归类为未知类型的分枝杆菌。 非感染性肉芽肿:结节病的诊断是基于临床与影像学检查结果一致、与组织学一致、特殊染色和微生物培养阴性的综合诊断。其他具体的诊断是基于既往综述中概述的标准组织学和临床标准。缺乏具体病因的临床、微生物学或组织学证据的病例被归类为病因不明的肉芽肿性炎症。每个参与者根据病理组织学结果和微生物培养、血清学和/或从现有病历前瞻性收集的临床研究中获得的所有可用数据对建立初步诊断。所有纳入患者均根据初步诊断进行相应的治疗并随访6个月,动态随访观察临床症状和影像学改善,如临床评估治疗无效或者恶化者进一步临床干预,并记录最终诊断。
