研究目的：

本项目研究拟计划对严重烧伤、真菌感染高风险患者采集的样本应用二代测序技术手段进行检测，将测序分析的结果与传统检测等方法进行比对，探讨NGS技术在真菌感染诊断中的灵敏度和准确性，为提高真菌感染诊断的准确率和提高患者的生存率提供有价值的研究结果；同时，对确诊感染患者的样本进行相关耐药基因的检测分析，可指导临床用药，制定个体化治疗方案，具有重要的理论和指导意义。  

Objectives of Study：

The project is planned to apply samples of second-generation sequencing to the samples collected from patients with high risk of severe burns and fungal infections, compare the results of sequencing analysis with traditional methods, and discuss the sensitivity of NGS technology in the diagnosis of fungal infections. And accuracy, providing valuable research results for improving the accuracy of fungal infection diagnosis and improving the survival rate of patients; at the same time, the analysis of related drug resistance genes in samples of patients with diagnosed infections can guide clinical medication and formulate individualization The treatment plan has important theoretical and guiding significance.

纳入标准：

① 年龄18-70岁，性别不限；
② 烧伤（火焰烧伤、电烧伤、化学烧伤、各种热液烫伤）总面积为30%及以上者；
③ 烧伤后24小时内入院者；
④ 无传染性疾病；
⑤ 签署同意书：试验前受试者均自愿签署知情同意书。

Inclusion criteria

1. Patients aged 18-70 years, regardless of gender;
2. Burn patients (flame burn, electric burn, chemical burn, various hydrothermal burns) with a total area of 30% or more;
3. Patients admitted to hospital within 24 hours after burn;
4. Patients without infectious diseases;
5. Signing consent: all subjects voluntarily signed informed consent before the experiment.

排除标准：

有严重免疫缺陷疾病，如艾滋病等患者。

Exclusion criteria：

Patients with severe immunodeficiency diseases, such as AIDS.

传统方法检测 包括（微生物学培养、组织病学检查、半乳甘露聚糖（GM）以及1，3-β-D葡聚糖（BDG）检测）

指标试验（即本研究的待评估诊断试验，无论为方法、生物标志物或设备，均请列出名称）：

a.宏基因组测序：采集患者疑似感染部位组织，分离提取DNA或采集患者和对照组健康人的血浆，分离血浆cell-free DNA，进行宏基因组测序，对数据进行分析比对；其中移除确定为人类的序列reads，剩下的序列则与病原体数据库进行mapping比对，研究鉴定微生物物种，种群结构等； b.18S及ITS测序：对采集的患者疑似感染部位组织进行分离提取DNA，设计特定引物进行18S、ITS测序。

1. Metagenome sequencing: collect tissues from suspected infection sites of patients, isolate DNA, or collect plasma from patients and healthy individuals, isolate plasma cell-free DNA, perform metagenomic sequencing, and analyze and compare data 2. 18S and ITS sequencing.

烧伤（火焰烧伤、电烧伤、化学烧伤、各种热液烫伤）总面积为30%及以上者 (依据2012年4月第28卷2期《中华烧伤杂志》发表的《烧伤侵袭性真菌感染诊断与防治指南（2012版）》将试验组分为三组，分别是确诊侵袭性真菌感染组、疑诊侵袭性真菌感染组以及无真菌感染组)。
