研究目的：

评价健康管理模式对绝经后女性未来10年心血管疾病（CVD）发病风险及心血管危险因素干预的效果，为制定绝经后女性CVD的防控对策提供理论依据和新思路。  

Objectives of Study：

Objective to evaluate the effect of health management model on the risk of cardiovascular disease (CVD) and cardiovascular risk factors in postmenopausal women in the next 10 years, so as to provide theoretical basis and new ideas for the prevention and control of postmenopausal women with CVD.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄40～70岁；
（2）绝经后女性：距离末次月经时间一年以上者；
（3）既往无心脑血管疾病：冠心病(冠状动脉供血不足、心绞痛、心肌梗死、冠心病猝死) 、脑血管疾病(短暂性脑缺 血发作、缺血性脑卒中)、外周动脉疾病(间歇性跛行)和心力衰竭。
（4）有一定认知能力，可独立完成相关问卷填写。

Inclusion criteria

(1) The age ranged from 40 to 70 years;
(2) Postmenopausal women: more than one year from the last menstrual period;
(3) No previous cardiovascular and cerebrovascular diseases: coronary heart disease (coronary artery insufficiency, angina pectoris, myocardial infarction, sudden death of coronary heart disease), cerebrovascular disease (transient ischemic attack, ischemic stroke), peripheral artery disease (intermittent claudication) and heart failure.
(4) Have certain cognitive ability, can complete the relevant questionnaire independently.

排除标准：

（1）有体力活动禁忌症者；
（2）既往有精神病史者；
（3）知情同意：不愿参加本次研究者。

Exclusion criteria：

(1) Those who have contraindications of physical activity;
(2) Patients with previous mental history;
(3) Informed consent: not willing to participate in this study.
