研究目的：

通过对脑卒中失能老年患者及其家属的同步递进式健康教育干预，观察老年患者从入院到出院后3个月内ADL、情绪、营养状况及血液指标等变化，来探索健康教育干预的最合适内容和时机，为脑卒中老人及家庭提供针对性的帮助。  

Objectives of Study：

Through to the disability elderly stroke patients and their families a type of progressive synchronization health education intervention, observation of elderly patients from hospital to ADL within 3 months after discharge, mood, and blood indexes such as nutritional status changes, to explore the most appropriate content of health education intervention and timing, for old people and families to provide targeted help stroke.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

患者的入选标准：①因脑卒中入院并导致ADL依赖，入院前2周ADL正常者；②年龄≥60周岁；③与家人共同居住；④对本调查知情同意。

照顾者的入选标准：①是病人家属或有亲缘关系，提供不计报酬的照顾服务；②为病人的主要家庭照顾者，以照顾时间最长为标准；③年龄≥18岁；④能够和研究者进行有效沟通，并且知情同意。

Inclusion criteria

Inclusion criteria of patients:
1. Admitted to a stroke due to stroke and caused ADL dependence, those with normal ADL 2 weeks before admission;
2. Aged >= 60 years;
3. Live with family members;
4. Informed consent to the investigation.

The inclusion criteria of caregivers:
1. Family members or relatives of patients who provide unpaid care services;
2. The patient's primary family caregiver, with the longest caring time as the standard;
3. Age ≥18 years old;
4. Able to communicate effectively with researchers, and informed consent.

排除标准：

患者的排除标准：①独居或未与家人共同居住；②智力障碍；③患严重的认知功能障碍；④精神疾病者。

照顾者的排除标准：①与患者或患者家属间存在雇佣关系者；②在患者出院后不与其共同居住。

Exclusion criteria：

Exclusion criteria for patients: ①Living alone or not living with family members; ②Mental retardation; ③suffer from severe cognitive impairment; ④The mentally ill.

Exclusion criteria for caregivers: ①those who have an employment relationship with the patient or the patient's family members; ②Do not live with the patient after discharge.
