研究目的：

内镜逆行性阑尾炎治疗术(ERAT)是一种新兴的内镜治疗急性非复杂性阑尾炎(AUA)的方法。然而，到目前为止，还没有对儿童进行系统的研究，缺乏前瞻性的比较数据。此外，由于担心电离辐射对儿童未来的恶性肿瘤风险，我们使用对比增强超声(CEUS)而不是内镜逆行阑尾造影(ERAR)。因此，我们进行了一项前瞻性随机对照临床试验，比较改良ERAT (mERAT)和抗生素治疗对AUA患儿的疗效。本研究的目的是评估mERAT治疗住院的AUA患儿的安全性和可行性。  

Objectives of Study：

Endoscopic retrograde appendicitis therapy (ERAT) is an emerging endoscopic treatment modality for acute uncomplicated appendicitis (AUA) supported by several case series. However, to date, systematic studies have not been conducted in children and the prospective comparative data are lacking. Moreover, due to a concern for future malignancy risk in children from ionizing radiation, we used contrast-enhanced ultrasound (CEUS) instead of endoscopic retrograde appendiceal radiography (ERAR). Therefore, we conducted a prospective, randomized control clinical trial to compare the modified ERAT (mERAT) to antibiotic therapy in children with AUA. The aim of this study was to evaluate the safety and feasibility and of mERAT in the treatment of hospitalized children with AUA.

纳入标准：

2018年10月至2020年2月间我院急诊科诊断的所有1 - 13岁的急性阑尾炎患儿，且患儿家属拒绝接受标准的外科手术。急性阑尾炎的诊断是基于急性腹痛和计算机断层(CT)和/或超声(US)发现阑尾肿大或阑尾伴粪石梗阻。

Inclusion criteria

We enclosed all children, at 1 to 13 years of age, with a diagnosis of acute appendicitis between October, 2018 and February, 2020 in our emergency department and familys refusal to have a standard surgical intervention. The diagnosis of acute appendicitis is based on acute abdominal pain and computed tomography (CT) (Fig 1a, b) and/or ultrasonography (US) (Fig 1c, d) finding of an enlarged appendix or obstruction of appendix with fecalith.

排除标准：

(1)阑尾穿孔、弥漫性腹膜炎或阑尾周围脓肿患儿;(2)显示腹腔内游离空气及泌尿、妇科等急腹症原因的腹部x线片;(3)有结肠镜检查禁忌症或对造影剂过敏的儿童及(4)家庭不愿接受内镜治疗。

Exclusion criteria：

(1) children with appendix perforation, diffused peritonitis or periappendiceal abscess; (2) Abdominal radiograph showing intra-abdominal free air and other causes of acute abdomen such as urological and gynecological diseases; (3) Children with a contraindication for colonoscopy or an allergy to contrast media and (4) Family is reluctant to accept endoscopic treatment.

该临床研究按1:1比例将受试者随机分为2组，A组：mERAT治疗；B组：抗生素治疗；列出流水号为01-74所对应的治疗分配（随机编码表），且流水号与受试者编号对应，随机编码由指定人员保管。受试者入选后，研究者将相应的受试者编号通知随机编码表保管者，由后者根据随机编码表下达该入选受试者应进入A组还是B组的指令，研究者接指令后应有相应的记录，并遵照指令实施相应的分配。
