研究目的：

评价不同通气模式下对老年患者腹腔镜结直肠癌手术视神经鞘直径的影响,进而为临床上该类患者提供更合适的通气选择。  

Objectives of Study：

To evaluate the effect of different ventilation modes on the optic nerve sheath diameter of elderly patients undergoing laparoscopic colorectal cancer surgery, so as to provide more suitable ventilation options for such patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）择期全麻下接受腹腔镜下结直肠癌根治术患者；
（2）年龄 60～80岁，性别不限；
（3）ASA 分级Ⅰ～II级；
（4）BMI：18～30kg/m2。

Inclusion criteria

1. Patients undergoing laparoscopic radical resection of colorectal cancer under selective general anesthesia;
2. Patients aged 60-80 years old;
3. ASA grade I-II;
4. BMI 1830kg/m2

排除标准：

（1）有颅内压增高综合征病史（颅内病变，脑血管病史等）；
（2）曾接受过眼科手术或有眼科疾病史（青光眼，糖尿病性视网膜病，白内障和视网膜脱离等）；
（3）严重心脏和肺部疾患(肺部感染，肺不张，慢性阻塞性疾病，哮喘等)；
（4）有精神疾病或拒绝参与试验者；

Exclusion criteria：

(1) A history of increased intracranial pressure syndrome (intracranial lesions, cerebrovascular disease, etc.); (2) Have undergone ophthalmic surgery or have a history of ophthalmic diseases (glaucoma, diabetic retinopathy, cataract, and retinal detachment, etc.); (3) Severe heart and lung diseases (pulmonary infection, atelectasis, chronic obstructive disease, asthma, etc.); (4) Those who have mental illness or refuse to participate in the trial;

麻醉诱导前10分钟（T0），仰卧位麻醉诱导后10 min（T1）,建立CO2气腹（12 mmHg的充气压力）陡峭Trendelenburg体位30 min后（T2）, CO2气腹陡峭Trendelenburg体位90 min后(T3), 以及手术结束时气腹放气后仰卧位10 min（T4）

测量方法：

ONSD测量由同一位受过眼超声培训的麻醉医生测量，其并不知患者的随机分组情况，使用一个7.5Hz线性探头涂抹耦合剂轻柔放于患者上眼睑，不能对眼睛施加太大压力，探头被调整成以二维模式显示进入眼球的条状低回声视神经鞘，分别在双眼的冠状面和矢状面上测量球后3mm处的ONSD取平均值。
