研究目的:

比较不同降糖药物对T2DM患者种植体骨结合早期边缘骨吸收的干预作用,并探究其中某种药物是否对种植体周围骨组织高度有更好的保护效果。一方面可以为T2DM患者在血糖控制良好的基础上选择有利于骨代谢的降糖药物和用药方式,充分发挥抗糖尿病药物的积极骨靶点效应。另一方面通过测量各组T2DM患者骨结合早期的种植体周围骨水平变化,可以与降糖药物的全身骨代谢效应形成参照,这也是本研究的临床意义之一。  

Objectives of Study:

This aim of this study is to compare the influence of different hypoglycemic drugs on the early marginal bone loss of implant in patients with T2DM, and explore whether one of the drugs had a better protective effect on the bone tissue around the implant. On the one hand, hypoglycemic drugs and medication methods that are beneficial to bone metabolism should be selected for T2DM patients on the basis of good blood glucose control, considering the positive bone target effect of anti-diabetes drugs. On the other hand, this study measures the changes of bone level around the implant in the early stage of osseointegration of T2DM patients in each group. It can be a reference to the systemic bone metabolism effect of hypoglycemic drugs, which is also one of the clinical significance of this study.

药物成份或治疗方案详述:

 

Description for medicine or protocol of treatment in detail:

 

纳入标准:

(1)术前缺牙时间大于3个月;(2)T2DM确诊2年以上且病程稳定;(3)随访自我报告血糖控制稳定(HbA1c< 8.8%);(4)控制血糖方式为二甲双胍、胰岛素或GLP-1类药物其中之一;(5)未使用骨增量手段。

Inclusion criteria

(1)the teeth missing time more than three months;(2)patients with medically diagnosed T2DM more than 2 years duration; (3)self-reported good blood glucose control(HbAlc<8.8%); (4) treatment with one of the medication: metformin, insulin and GLP-1 drugs; (5)no use of bone augmentations

排除标准:

(1)骨质疏松或长期使用糖皮质激素、双膦酸盐等药物者;(2)血糖控制不良(HbA1c>8.8%)或具有严重的急性糖尿病并发症或器质性病变,如心脑血管及双下肢血管病变、糖尿病肾病、视网膜病变及神经系统并发症;(3)吸烟、嚼槟榔等不良习惯者;(4)随访影像资料不完整者。

Exclusion criteria:

(1) osteoporosis or long-term use of bisphosphonates or steroids; (2) poor glycemic control or severe diabetic complications, such as cardiovascular and cerebrovascular diseases and lower extremity vascular diseases, diabetic nephropathy, retinopathy and neurological complications; (3)excessive tobacco use and alcohol consumption; (4)patients whose records were incomplete.
