研究目的：

本研究旨在通过对食管裂孔疝采用完全左侧手术入路与传统手术入路（即双侧手术入路）进行优效性、随机对照研究，比较两种不同手术方式对于食管裂孔疝患者近、远期疗效和围手术期安全性的影响，最大程度进行器官功能和神经的保护，降低并发症的发生，提高患者生活质量，为规范食管裂孔疝的外科治疗提供证据支持。  

Objectives of Study：

The purpose of this study is to conduct a superior, randomized controlled study on the complete left-side surgical approach and traditional surgical approach (ie, bilateral surgical approach) for esophageal hiatal hernia, and compare the two different surgical methods for patients with esophageal hiatal hernia. , The long-term efficacy and the impact of perioperative safety, maximize the protection of organ function and nerves, reduce the occurrence of complications, improve the quality of life of patients, and provide evidence support for the standardization of surgical treatment of esophageal hiatal hernia.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①经胃镜及食管高分辨测压、24小时食管pH检测、上消化道造影、腹部CT检查均支持食管裂孔疝（II\III\IV型）伴有GERD；
②患者有反流、反酸、胸前区烧灼痛等返流症状，且经过规律的内科保守治疗症状无明显好转；
③年龄18-75岁，无论性别，体力评分ECOG≤2分，ASA评分≤2分，可耐手术治疗，肝、肾、骨髓功能正常；
④能够遵守研究期间的方案，并签署知情同意书；
⑤患者愿意接受随访并配合专职人员做好相关记录；
⑥无胃肠道手术史、恶性肿瘤病史、无化疗或放疗史。

Inclusion criteria

①Through gastroscopy and high-resolution esophageal manometry, 24-hour esophageal pH test, upper gastrointestinal angiography, and abdominal CT examination support esophageal hiatal hernia (type II\III\IV) with GERD;
②The patient has reflux symptoms such as reflux, acid reflux, and burning pain in the chest area, and the symptoms have not improved significantly after regular medical conservative treatment;
③Age 18-75 years old, regardless of gender, physical performance score ECOG ≤ 2 points, ASA score ≤ 2 points, resistant to surgical treatment, normal liver, kidney and bone marrow function;
④ Ability to abide by the plan during the study and sign an informed consent;
⑤ The patient is willing to follow up and cooperate with the full-time staff to make relevant records;
⑥ No history of gastrointestinal surgery, malignant tumor history, no history of chemotherapy or radiotherapy.

排除标准：

①合并严重心肺疾病且对手术无法耐受的患者；
②不愿意承担手术风险，未能签署手术知情同意书的患者；
③不能控制的内科疾病（包括6个月内出现的不稳定心绞痛、心肌梗塞、脑血管意外）；
④因全身其他脏器情况不能接受全麻或手术治疗；
⑤有未控制的癫痫、中枢神经系统疾病或精神障碍史者；
⑥器官移植需要免疫抑制治疗者；
⑦严重的未控制的反复感染者，或其它严重的未控制的伴随疾病；
⑧持续全身类固醇激素治疗；
⑨妊娠或哺乳期妇女育龄妇女；
⑩同时参加其它临床试验。

Exclusion criteria：

① Patients with severe cardiopulmonary disease and intolerance to surgery;
②Patients who are unwilling to bear the risk of surgery and fail to sign the informed consent for surgery;
③ Uncontrollable medical diseases (including unstable angina pectoris, myocardial infarction, cerebrovascular accident within 6 months);
④Cannot receive general anesthesia or surgical treatment due to other organs in the whole body;
⑤Have a history of uncontrolled epilepsy, central nervous system diseases or mental disorders;
⑥ Those who need immunosuppressive treatment for organ transplantation;
⑦Severe uncontrolled repeated infections, or other serious uncontrolled concomitant diseases;
⑧Continuous systemic steroid hormone treatment;
⑨Pregnant or breastfeeding women of childbearing age;
⑩ Participate in other clinical trials at the same time.
