研究目的：

中风后患者常遗留多种功能障碍,手功能障碍是中风后肢体康复的难点，手部的恢复通常比其他部位迟缓，延缓了偏瘫患者康复进程。对应取穴法在《内经》的“巨刺”“缪刺”及“远道刺”等取穴方法引申发展而成。笔者在临床中发现，在患者健侧足部穴位上针刺，即通过对应取穴，并加入互动式针刺，嘱患者主动活动手部，往往在针刺的同时就可以取得一定疗效，不亚于患侧手部局部取穴效果。本研究采用单中心随机对照临床试验，以中风后手功能障碍患者为研究对象，对照组采用醒脑开窍针刺法，观察组采用对应互动取穴法配合醒脑开窍针刺法，以简化 Fugl-Meyer 运动功能评分量表（FMA）手指评分以及握力、关节活动度等指标对两组治疗前后手功能情况进行评定评价，为针刺治疗缺血性中风恢复期患者手功能障碍进行有益探索。  

Objectives of Study：

Patients often suffer from multiple dysfunctions after stroke. Hand dysfunction is the difficulty of limb rehabilitation after stroke. Hand recovery is usually slower than other parts, which delays the rehabilitation process of patients with hemiplegia. Corresponding to the method of acupoints development in the "Nei Jing", such as "giant thorn", "miao thorn" and "distant thorn". The author found in the clinic that acupuncture is performed on the acupuncture points on the contralateral side of the patient, that is, through corresponding acupuncture and interactive acupuncture, the patient is instructed to actively move the hand. Often, a certain effect can be achieved at the same time as acupuncture. Sub-acupoint effect on the affected hand. This study used a single-center randomized controlled clinical trial. Patients with hand dysfunction after stroke were taken as the research object. The control group used the acupuncture method of Xingnaokaiqiao acupuncture, and the observation group used the corresponding interactive acupoint method combined with Xinaokaiqiao acupuncture to simplify Fugl -Meyer Motor Function Rating Scale (FMA) finger score, grip strength, joint mobility and other indicators were used to evaluate and evaluate the hand function before and after treatment in both groups, which is a useful exploration for acupuncture treatment of hand dysfunction in patients with ischemic stroke recovery.

纳入标准：

①首次发病或既往有中风病史但无后遗症，符合缺血性中风诊断标准，并经过CT 或 MRI 确诊者；
②偏瘫侧手功能Brunnstrom分级为Ⅱ-Ⅳ；
③病程：发病14天到3个月；
④年龄在 35～80 岁，男女均可；
⑤受试者知情同意，并签署知情同意书。

Inclusion criteria

① First onset or previous history of stroke but no sequelae, meeting the diagnostic criteria for ischemic stroke, and confirmed by CT or MRI;
② Brunnstrom classification of hemiplegia lateral hand function is Ⅱ-Ⅳ ;
③ course of disease: 14 days to 3 months;
④Age is 35 80 years old, both men and women;
⑤ Subjects' informed consent and signed informed consent.

排除标准：

①合并其他手腕及手指关节疾病者；
②脑卒中后神志障碍，如昏睡、昏迷不能回答问题，执行指令者；或严重失语，无言语或听力理解能力者。
③合并严重心功能不全，肝、肾功能严重异常和造血系统疾病，精神病患者。
④妊娠或准备妊娠妇女，哺乳期妇女。

Exclusion criteria：

①Patients with other wrist and finger joint diseases;
②Patients with mental disorders after stroke, such as lethargy, coma who cannot answer questions and execute instructions; or severe aphasia, speech or hearing comprehension.
③ Patients with severe heart dysfunction, severe abnormalities in liver and kidney function, hematopoietic system diseases, and mental illness.

基础治疗+醒脑开窍针刺法+对应互动取穴法治疗，具体方案如下： 对应互动取穴法选取健侧下肢太溪、解溪、太白、束骨等对应穴位，针刺深度10-15mm，采用提插泻法，针刺同时要求患者主动或者被动活动患侧手腕及手指，后留针30分钟。

两组按照1:1比例，由不参与研究的研究护士独立使用计算机Excel软件产生随机分配序列，分配序列单数为针刺组，双数为对照组。产生的随机分配序列被放入按顺序编码、密封、不透光的信封，当研究人员确定受试对象的合格性后，按顺序拆开信封并将受试对象分配入针刺组或者药物组。为最大限度减少偏倚，信封内的分配序列卡片为力敏型记录纸，研究者在拆开信封前将合格受试对象的姓名写在信封表面。
