研究目的：

主要目的 评估与传统锁骨下中心静脉置管相比，评价经锁骨下静脉放置带皮下隧道的PICC管是否能减少导管相关感染发生率：每1000个置管日发生导管相关性感染的例数。 次要目的 1）导管相关血栓发生率 2）导管相关机械损伤并发症发生率：刺破动脉、气胸、血胸、纵膈血肿、导管尖端位置错误、局部血肿或出血、空气栓塞。 3）建立中心静脉导管的手术时间。 4）测量中心静脉压的准确度  

Objectives of Study：

The main objective of study: Compare the rate of catheter-related infection, including catheter-related bacteraemia and non-bacteraemic catheter-related sepsis, between two groups: Incidence rates of catheter-related infection and colonisation are reported per 1000 catheter-days A secondary purpose: 1) incidence of catheter related thrombus 2) mechanical complications: arterial puncture, pneumothorax, hemothorax, mediastinal hematoma, wrong location of tip of catheter , local hematoma or hemorrhage, air embolism. 3) operational time to establish the central venous catheter. 4) accuracy of measurement of central venous pressure

药物成份或治疗方案详述：

试验组：经锁骨下静脉放置带皮下隧道的PICC管。对照组：标准锁骨下中心静脉置管。 

Description for medicine or protocol of treatment in detail：

subcutaneous tunneled subclavian venous catheterization AND Non-tunneled Central venous catheterization 

纳入标准：

1、年龄18-65岁，男女不限；
2、第一次放置锁骨下中心静脉管；
3、导管置入锁骨下静脉；
4、预期留置中心静脉导管时间>=3天
5、病人/家属知情并自愿签署知情同意书。

Inclusion criteria

1. 18-65 years old, male or female;
2. The subclavian central vein is placed for the first time;
3. Catheter was inserted into the subclavian vein;
4. The time of keeping central venous catheter is >=3 days
5. The patient/family is informed and voluntarily signs the informed consent.

排除标准：

1、导管不置入锁骨下静脉；
2、穿刺前穿刺部位皮肤发炎；
3、试验开始前已知对任何药物过敏；
4、参与另一项临床试验；
5、现场紧急插入导管；
6、中央静脉出现明显血栓/狭窄
7、血小板计数<50×10 3/μL，凝血酶原时间>正常范围的1.6倍，部分凝血活酶时间>正常范围的2倍，或采用治疗性抗凝
8、严重低氧血症（氧合指数<150mmHg）
9、男的体重指数>35kg/m2，女的体重指数>30kg/m2；
10、无法签署知情同意书
11、意向妊娠或育龄妇女妊娠试验阳性及哺乳期妇女；
12、研究者认为不适宜参加临床试验的患者。

Exclusion criteria：

1. The catheter is not inserted into the subclavian vein;
2. The skin at the puncture site is injured before puncture;
3. Known allergy to any drug before the trial;
4. Participate in another clinical trial;
5. Emergency catheter insertion;
6. Obvious thrombus/stenosis of the central vein
7. Platelet count <50×10 3/u L, prothrombin time >1.6 times of the normal range, and partial thrombin activity time >2 times of the normal range, or have therapeutic anticoagulation
8. Severe hypoxemia (oxygenation index <150mmHg)
9. Bmi of >35kg/m2 for male and >30kg/m2 for female;
10. Unable to sign the informed consent
11. Positive pregnancy test for women or lactating women;
12. Patients considered inappropriate by the investigator to participate in clinical trials.^
