研究目的：

评估八式太极拳对脑小血管病认知功能障碍患者认知功能以及脑血管病高危因素的影响。同时采用fMRI和事件相关电位等方法探讨其可能机制。  

Objectives of Study：

To evaluate the modulation effect of 24-week eight-style Tai Chi exercise in patients with cognitive impairment of cerebral small vessel disease, and explore the underlying mechanism based on fMRI and event-related potential(ERP).

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 符合脑小血管病相关认知功能障碍西医诊断标准（需有脑小血管病的影像学证据并完善认知功能筛查、评估等，具体参考《脑小血管病相关认知功能障碍中国诊疗指南（2019）》）；
2. MoCA 量表评分19～25分；
3. 年龄≥50岁；
4. 最近半年内未从事有规律的运动锻炼（即每周至少3次，每次20分钟的各种形式的规律性锻炼）；
5. 受试者本人已签署知情同意书；

Inclusion criteria

1. Match the diagnostic criteria of cognitive impairment of cerebral small vessel disease（according to Clinical practice guideline for cognitive impairment of cerebral small vessel disease of China(2019)）;
2. MoCA score: 19-25;
3. aged 50 years or older;
4. no regular physical exercise for at least half a year(regular exercise means exercise with a frequency of at least three times a week and at least 20 min per session);
5. The subject has signed informed consent;

排除标准：

1. 脑脱髓鞘引起的脑白质疏松；大动脉粥样硬化软斑；皮质及分水岭梗死，皮质下直径大于 15mm 的病灶；
2. 非小血管的认知功能障碍的其他脑部病变，如：如阿尔茨海默病、路易体痴呆、额颞叶痴呆、帕金森病、肿瘤、脑积水、外伤、梅毒、获得性免疫缺陷综合征、克雅氏病等；
3. 皮质和（或）皮质下非腔隙性梗死病灶、出血性病灶；
4. 脑白质病变特殊原因(多发性硬化、结节病、脑部放疗) ；
5.患有严重脏器衰竭、严重的心脑血管疾病、肌肉骨骼系统疾病以及其他运动禁忌症而不适宜进行太极拳运动者；
6.经既往病历或临床医生或家属证实其之前已经患有认知障碍的相关疾病或在发病前有使用针对认知障碍的药物。
7. 痴呆（GDS等级II~III级）, 生活不能自理(ADL评分＜20分)，严重精神疾病、癫痫, 严重抑郁症（贝克抑郁量表＞10 分）等。
8.酒精及药物滥用史。
9.MRI检查禁忌症者
10.正在参加影响本研究结果评价的其它临床试验者。
符合以上任何一条标准者，不予纳入研究。

Exclusion criteria：

1. Leukopenia caused by demyelination of the brain; aortic atherosclerotic soft spots; cortical and watershed infarcts; lesions with a subcortical diameter greater than 15mm;
2. There are other causes of cognitive impairment, such as Alzheimer's disease, Lewy body dementia, frontotemporal dementia, Parkinson's disease, tumors, hydrocephalus, trauma, syphilis, acquired immune deficiency syndrome, Creutzfeldt-Jakob disease;
3. Cortical and/or subcortical non-lacunar infarcts, hemorrhagic lesions;
4. White matter lesions caused by other diseases such as multiple sclerosis, Sarcoidosis, brain radiotherapy;
5. Current severe medical conditions for which exercise is contraindicated, such as severe cardiovascular, renal, hepatic, musculoskeletal diseases;
6. Previous medical records or clinicians or family members have confirmed that they have previously suffered from cognitive impairment-related diseases or have used drugs for cognitive impairment before onset;
7. Dementia (GDS grades II-III), can not take care of themselves（(ADL Scale＜20points)）, severe mental illness, epilepsy, severe depression (Baker Depression Scale> 10 points);
8. History of alcohol intoxication, drug addiction;
9. Contraindication for MRI examination;
10. Participating in other clinical trials that influence the evaluation of the results of this study;
