研究目的：

本研究拟对非机械通气严重低氧的新型冠状病毒肺炎患者采用俯卧位，观察10分钟、30分钟后患者呼吸频率、血氧饱和度改善情况。通过分析患者肺部影像学改善情况，进一步定性证实俯卧位对新冠肺炎的改善情况，并探讨患者是否实行俯卧位以及患者实行俯卧位的依从度与进一步呼吸支持的需求程度和预后的关系。  

Objectives of Study：

In this study, newly admitted COVID-19 patients with severe hypoxia were placed in the prone position group and non-prone position group, and the respiratory rate and blood oxygen saturation of the patients were observed after 10 minutes and 30 minutes.By analyzing the improvement of the patient's pulmonary imaging, the improvement of the prone position on covid-19 was further confirmed qualitatively, and the relationship between the patient's implementation of the prone position and the patient's compliance with the implementation of the prone position, the need for further respiratory support and the prognosis was explored.

药物成份或治疗方案详述：

俯卧位规范流程：患者符合入组条件，如果患者体质虚弱，呼吸频促，就建议患者坐在床旁的凳子上，然后尝试趴在床上；如果病情允许能配合的情况下，则建议患者双手抱肘，俯卧位趴在床上。10分钟后、30分钟后分别记录患者呼吸频率、血氧饱和度、心率和血压等情况。分别由两名医生评估患者对于俯卧位的执行情况，确定安全可靠性。建议患者每次1-2个小时，3-4次/日，连续5天。俯卧位总时间超过3h则开始入组追踪，每日记录俯卧位总时间，如患者在期间发生呼吸支持措施升级（采用经鼻高流量、无创正压通气、气管插管、气管切开）则终止清醒俯卧位。并继续追踪随访患者进一步呼吸机支持需求及预后。 

Description for medicine or protocol of treatment in detail：

Standardized prone position process: Patients in the group meet the requirements of enrollment. If the patient is weak and breathing frequently, it is recommended that the patient sit on the bench beside the bed and then try to lie on the bed prone. If the patient is allowed to cooperate, it is recommended that the patient hold his elbow with both hands and lie on the bed prone. Respiratory rate, oxygen saturation, heart rate and blood pressure were recorded after 10 minutes and 30 minutes respectively. Two physicians assessed the patient's performance in the prone position to determine safety and reliability. Patients are advised to spend 1-2 hours each, 3-4 times/day for 5 consecutive days. When the total prone position time was more than 3 hours, group tracking was started, and the total prone position time was recorded daily. If the patients had escalation of respiratory support measures (using nasal high flow, noninvasive positive pressure ventilation, endotracheal intubation, tracheotomy) during the period, the awake prone position was terminated. Further ventilator support needs and prognosis were followed up. 

纳入标准：

1、2019-nCoV核酸检测阳性，并具有明显的病毒性肺炎临床特征；2、严重缺氧，需要使用储氧面罩吸氧支持，脉搏氧饱和度能维持在90%左右；3、年龄18-80岁，具有自主活动能力的患者；4、本人或委托代理人签署知情同意书。

Inclusion criteria

1) The COVID-19 patient is diagnosis by nucleic acid detection, who has obvious clinical features of viral pneumonia; 2) The COVID-19 patient with severe hypoxia requires the use of oxygen storage masks, and whose pulse oxygen saturation can be maintained at about 90%; 3) The COVID-19 patient has the ability to move independently with age range from 18-80 years old; 4) Informed consent form of COVID-19 patient is signed by myself and agent.

排除标准：

1、存在无法耐受俯卧位或俯卧位极易造成伤害的情况，比如神经、精神问题，不能自主活动；尚未稳定的脊髓损伤或骨折(椎体骨折、骨盆骨折、多发骨折、连枷胸等)；未缓解的颅内压升高；严重的烧伤；腹部手术后；腹腔高压；头面部损伤；气管切开；孕产妇。2、严重慢性肺病。3、血流动力学不稳定或合并其他器官功能衰竭需 ICU 监护治疗。4、临床预判无生存希望，仅进行临终关怀的病例，或者深度昏迷且在入院三小时内对支持治疗措施无反应的病例。

Exclusion criteria：

1. conditions that cannot be tolerated in prone positioning or prone positioning can easily cause injury, such as having neurological or mental problems, and inability to move autonomously; 2.Spinal cord injury or fracture (vertebral body fracture, pelvic fracture, multiple fractures, flail chest, etc.) that has not yet stabilized;Increased intracranial pressure without relief; Severe burned; Recently abdominal surgery; Abdominal hypertension; Head and face injuries; Tracheotomy; Maternal.2. Severe chronic lung disease.3. Unstable hemodynamic status or other organ failure should be treated in ICU.4. Patients with no hope of survival and only hospice care were clinically predicted, or those who were in a deep coma and did not respond to the supportive treatment measures within three hours after admission.
