研究目的：

本研究以国家慢性病管理政策为指导，采用前瞻性和回顾性研究相结合、质性和量性研究相结合的原则，对脑卒中高危人群的健康管理关键指标进行筛选和量化，构建健康管理模式。从而降低脑卒中的发病率和复发率，提高脑卒中患者及高危人群的生活质量，为脑卒中高危人群的健康管理提供新思路，并为卫生发展政策规划和改进提供依据。  

Objectives of Study：

Guided by the national chronic disease management policy, this study adopted the principle of combining prospective and retrospective studies, and combining qualitative and quantitative studies to select and quantify the key indicators of health management for high-risk groups of stroke, so as to construct a health management model. In this way, the morbidity and recurrence rate of stroke can be reduced, and the quality of life of stroke patients and high-risk groups can be improved, providing new ideas for the health management of high-risk groups of stroke, and providing a basis for the planning and improvement of health development policies.

纳入标准：

1.符合脑卒中高危人群标准者；2.能正常沟通交流者；3.年龄≥40岁者。

Inclusion criteria

1. Meet the criteria for high-risk groups of stroke; 2. Able to communicate normally; 3. Age ≥40 years old.

排除标准：

合并严重急慢性疾病，如严重的心力衰竭、肾功能衰竭、肝脏疾病，恶性肿瘤等。

Exclusion criteria：

Severe acute and chronic diseases, such as severe heart failure, renal failure, liver disease, malignant tumor, etc.

采用分层随机整群抽样的方法，随机抽取郑州市6个行政区域中原区、二七区、管城区、金水区、上街区、惠济区中的3个。为了保证干预效果，每个社区内按同法随机选择两个居民小区设置平行对照，分别设为干预组和对照组。
