研究目的：

探讨医共体管理模式下基层医院妊娠糖尿病患者血糖管理水平，及对妊娠结局的影响。  

Objectives of Study：

Study on blood glucose management level and pregnancy outcome of gestational diabetes patients in primary hospitals under the medical community management mode

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 符合《妇产科学》中关于妊娠期糖尿病的诊断标准；
2. 对本次研究内容知情同意，签署知情同意书；
3. 初中及以上文化程度，具备基本的沟通和交流能力；
4. 知情同意并自愿配合主管医师和护理人员的全程连续护理干预，愿意坚持门诊定期产检、住院治疗和随访.

Inclusion criteria

1. The patients who meet the diagnostic criteria for gestational diabetes in Obstetrics and Gynecology;
2. The patients who sign the informed consents;
3. The patients with junior high school degree or above, and basic communication ability;
4. The patients who can voluntarily cooperate with the the competent physicians and nursing staff for the full-time continuous nursing intervention, and are willing to insist on regular outpatient inspection, hospitalization and follow-up.

排除标准：

1. 多胎妊娠孕妇；
2. 瘢痕子宫孕妇；
3. 慢性高血压患者；
4. 入组前接受胰岛素治疗的患者；
5. 机体存在其他妊娠合并症及严重器质性病变的患者。

Exclusion criteria：

1. Pregnant women with multiple pregnancies;
2. Pregnant women with scarred uterus;
3. Patients with chronic hypertension;
4. Patients receiving insulin therapy before enrollment;
5. Patients with other pregnancy complications and severe organic lesions.
