研究目的：

1. 观察不同剂量镇痛方案的酒石酸布托啡诺用于下腹部肿瘤择期手术患者围术期的镇痛镇静效果 2. 观察不同剂量镇痛方案的酒石酸布托啡诺用于下腹部肿瘤择期手术患者围术期的不良反应发生情况。 3. 总结出酒石酸布托啡诺用于下腹部肿瘤择期手术患者围术期的安全有效的临床用药方案、并建立专家共识。  

Objectives of Study：

1. To observe the analgesic and sedative effects of butorphanol tartrate in different doses on patients undergoing selective operation of lower abdominal tumor 2. To observe the adverse reactions of different doses of butorphanol tartrate in patients undergoing elective surgery for lower abdominal tumor. 3. To sum up the safe and effective clinical medication plan of butorphanol tartrate in the peri operation of patients with lower abdominal tumor, and to establish the consensus of experts.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1）18-60岁，接受下腹部（结直肠肿瘤、泌尿科肿瘤、妇科肿瘤）肿瘤择期手术患者
2）ASA I-III级
3）术后镇痛使用电子自控静脉镇痛泵，且镇痛泵用药中包含酒石酸布托啡诺；
4）围手术期使用酒石酸布托啡诺镇痛镇静；
5）患者或家属已签署知情同意书；

Inclusion criteria

1. Patients aged 18-60 years who underwent selective surgery for tumors of the lower abdomen (colorectal cancer, urological tumor, gynecological tumor);
2. ASA level I-III;
3. Patients who use electronic controlled intravenous analgesia pump for postoperative analgesia, and the analgesic pump contains butorphanol tartrate;
4. Patients who used butorphanol tartrate for analgesia and sedation in perioperative period;
5. The patient or family member has signed the informed consent;

排除标准：

阿片耐受的患者；
依赖那可汀患者；
妊娠及哺乳期妇女；
有精神类疾病或其他原因无法配合者；
术前患有中枢神经系统疾病；颅脑损伤或颅内压升高的患者；
慢性疼痛病史；术前长期服用镇静药、抗抑郁药；酗酒；
同时服用兴奋CNS药或患有CNS疾病或呼吸功能缺陷的患者；
心肌梗塞、心室功能障碍、冠状动脉功能不全的患者；
近三月内参加其他药物临床研究者；
手术范围涉及膈肌的手术（卵巢癌瘤体减灭术）；
肌酐清除率＜80ml/min患者；
年龄小于18周岁患者。

Exclusion criteria：

1. Patients with opioid tolerance;
2. Patients who depend on nacodine;
3. Pregnant and lactating women;
4. Patients with mental illness or other reasons unable to cooperate;
5. Patients with central nervous system disease, craniocerebral injury or intracranial pressure rise before operation;
6. Patients with a history of chronic pain, long-term use of sedatives and antidepressants before operation; alcoholism;
7. Patients who take CNS stimulants or suffer from CNS disease or respiratory dysfunction at the same time;
8. Patients with myocardial infarction, ventricular dysfunction and coronary insufficiency;
9. Patients who participated in clinical studies of other drugs in recent three months;
10. The scope of operation involves diaphragm operation (reduction of ovarian cancer);
11. Patients with creatinine clearance rate < 80ml / min;
12. Patients younger than 18 years old.
