Objectives of Study：

In order to explore the effect of intelligent ankle-foot orthosis for children with cerebral palsy based on cloud platform on lower limb motor dysfunction in brain patients

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①符合中国脑性瘫痪康复指南(2015)制定的诊断标准，排外其他诊断，明确诊断为脑性瘫痪的患者。
②临床分型中主要为以下三类之一：
痉挛型四肢瘫（spastic quadriplegia） 以锥体系受损为主，包括皮质运动区损伤。牵张反射亢进是本型的特征。四肢肌张力增高，上肢背伸、内收、内旋，拇指内收，躯干前屈，下肢内收、内旋、交叉、膝关节屈曲、剪刀步、尖足、足内外翻，拱背坐，腱反射亢进、踝阵挛、折刀征和锥体束征等。
痉挛型双瘫（spastic diplegia） 症状同痉挛型四肢瘫，主要表现为双下肢痉挛及功能障碍重于双上肢。
痉挛型偏瘫（spastic hemiplegia） 症状同痉挛型四肢瘫，表现在一侧肢体。小腿三头肌及踝关节张力较高者；服药六个月以上且处于癫痫稳定期者
③粗大运动功能分级(GMFCS分级)为I~II级，GMFCS分级I~II级标准如下：
年龄2-4岁
Ⅰ级：孩子可以坐在地板上双手玩东西。他们可以在没有大人帮助下完成地板上坐位和站立位的姿势转换，孩子把行走作为首选移动方式，并不需要任何助步器械的帮助。
Ⅱ级：孩子可以坐在地板上，但当双手拿物体的时候可能控制不了平衡。他们可以在没有大人帮助的情况下自如地坐位转换。可以拉着物体站在稳定的地方。可以用手和膝交替爬行，可以扶着家具慢慢移动，他们首选的移动方式是使用助步器行走。
年龄4－6岁
Ⅰ级：孩子可以在没有双手帮助的情况下坐上、离开或者坐在椅子上。可以在没有任何物体支撑的情况下从地板上或者从椅子上站起来，他们可以在室内室外走动，还能爬楼梯，正在发展跑和跳的能力。
Ⅱ级：孩子可以在双手玩东西的时候在椅子上坐稳，可以从地板上或者椅子上站起来，但是经常需要一个稳定的平面供他们的双手拉着或者推着。可以在室内没有任何助行器的帮助下行走，在室外的水平地面上也可以走上一小段距离。他们可以扶着扶手爬楼梯，但是不能跑和跳。
年龄6－12岁
Ⅰ级：孩子可以没有任何限制地在室内和室外行走并且可以爬楼梯。他们能表现出跑和跳等粗大运动能力，但是他们的速度、平衡和协调能力都有所下降。
Ⅱ级：孩子可以在室内和户外行走，能够抓着扶手爬楼梯，但是在不平的地面或者斜坡上行走就会受到限制，在人群中或者狭窄的地方行走也受到限制。他们最多能勉强达到跑和跳的水平。
④ 3岁≦年龄≦12岁，能主动配合简单指令
⑤小腿三头肌及胫骨前肌肌肉张力改良Ashworth评分I~II级；
⑥患儿家长对本治疗方法知情同意，能配合治疗1个月以上
⑦自愿参加本研究并签署知情同意书，且获得临床医师准许。

Inclusion criteria

① It meets the diagnostic criteria set by the Chinese Guidelines for Rehabilitation of Cerebral Palsy (2015), excludes other diagnoses, and clearly diagnoses patients with cerebral palsy.②The clinical classification is mainly one of the following three categories:Spastic quadriplegia is mainly due to damage to the cone system, including damage to the cortical motor area. Hypertension stretch is a feature of this type. Increased muscle tone in the extremities, back extension, adduction, and internal rotation of the upper extremity, adduction of the thumb, forward flexion of the trunk, adduction of the lower extremity, internal rotation, crossover, knee flexion, scissor step, pointed foot, valgus, and arched back Tendon reflex hyperactivity, ankle clonus, clasp sign and pyramid beam sign.Spastic diplegia (spastic diplegia) The symptoms are the same as spastic diplegia, mainly manifested as spasticity and dysfunction of the lower limbs than the upper limbs.Spastic hemiplegia (spastic hemiplegia) Symptoms are the same as spastic quadriplegia, manifested in one limb. Those with high calf triceps and ankle tension; those who have been on medication for more than six months and are in a state of seizure stability③Gross motor function classification (GMFCS classification) is I ~ II grade, GMFCS classification I ~ II grade standards are as follows:Age 2-4 yearsLevel 1: Children can sit on the floor and play with their hands. They can complete the posture transition between sitting and standing positions on the floor without the help of an adult. Children use walking as their preferred method of movement and do not need the help of any walking aids.Level Ⅱ: The child can sit on the floor, but may not be able to control the balance when holding the object with both hands. They can freely change positions without the help of an adult. You can pull the object and stand in a stable place. You can crawl alternately with your hands and knees, and you can slowly move the furniture, and their preferred method of movement is to walk with a walker.Age 4-6 yearsLevel I: The child can sit, leave, or sit in a chair without the help of both hands. They can stand up from the floor or from a chair without any objects supporting them. They can walk around indoors and outdoors, climb stairs, and are developing the ability to run and jump.Level II: Children can sit firmly on a chair while playing with their hands, and can stand up from the floor or chair, but often need a stable surface for their hands to pull or push. You can walk indoors without the help of any walkers, or walk a short distance on the outdoor horizontal ground. They can support the handrail to climb the stairs, but they cannot run and jump.Age 6-12Level I: Children can walk indoors and outdoors without any restrictions and can climb stairs. They can show gross athleticism such as running and jumping, but their speed, balance, and coordination have all declined.Level II: Children can walk indoors and outdoors, and can climb stairs by holding handrails, but walking on uneven ground or slopes is restricted, and walking in crowds or narrow places is also restricted. They can barely reach the level of running and jumping.④ 3 years old ≦ age ≦ 12 years old, can actively cooperate with simple instructions⑤ Modified Ashworth score I ~ II for the calf triceps and tibialis anterior;⑥The parents of the child have informed consent to this treatment method and can cooperate with the treatment for more than 1 month⑦ Voluntarily participate in this study and sign the informed consent form, and obtain the permission of the clinician.

排除标准：

①不符合中国脑性瘫痪康复指南(2015)制定的诊断标准且并伴有严重认知、感觉障碍、对电流过敏等并发症患者,癫痫近期发作期者
②符合中国脑性瘫痪康复指南(2015)制定的诊断标准，但不属于痉挛型四肢瘫、痉挛性双瘫和痉挛性偏瘫的患儿
③近一年内进行过下肢矫形手术者；或最近3个月参加过其他的临床试验
④有其他神经系统疾病、前庭功能或小脑功能障碍者，进行性重疾病患者儿；或严重的合并症及并发症
⑤年龄小于3岁，或大于12岁
⑥不配合治疗或存在康复训练禁忌症的患儿
⑦家长或监护人不同意治疗的患儿
⑧排除有严重长短腿及合并其他步态问题的病例

Exclusion criteria：

① Patients who do not meet the diagnostic criteria established by the Chinese Guidelines for Rehabilitation of Cerebral Palsy (2015) and are accompanied by complications such as severe cognition, sensory disturbances, and current allergies, and those with recent seizure②Children who meet the diagnostic criteria stipulated by the Chinese Guidelines for Rehabilitation of Cerebral Palsy (2015), but are not children with spastic quadriplegia, spastic diplegia and spastic hemiplegia③Those who have undergone orthopedic surgery within the past year; or have participated in other clinical trials in the last 3 months④ Patients with other neurological diseases, vestibular function or cerebellar dysfunction, patients with progressive severe disease; or severe comorbidities and complications⑤Age younger than 3 years old, or older than 12 years old⑥Children who do not cooperate with treatment or have contraindications for rehabilitation training⑦Children whose parents or guardians do not agree to treatment⑧Exclude cases with serious long and short legs and other gait problems
