评价面部加压绷带包扎在复杂埋伏阻生智齿拔除术后对患者面部肿胀、张口受限等术后反应的临床应用效果。  

Objectives of Study：

Evaluate the clinical application effect of facial compression bandaging on postoperative reactions such as facial swelling and limited mouth opening in patients after extraction of complex impacted wisdom teeth.

纳入标准：

选取于广州医科大学附属口腔医院越秀院区口腔颌面外科就诊并要求拔除下颌阻生智齿的患者; 纳入标准： 1.性别不限； 2.年龄17~40岁； 3.影像学检查属于Pell & Gregory分类的II~III类，中、低位下颌埋伏阻生智齿； 4.临床检查评估拔牙手术需切开翻瓣、去骨暴露智齿。

Inclusion criteria

Patients who visited the Department of Oral and Maxillofacial Surgery of the Yuexiu Branch of Guangzhou Medical University Affiliated Hospital and requested the extraction of impacted wisdom teeth in the mandible were selected; Inclusion criteria: 1. No gender restrictions; 2. Age ranging from 17 to 40 years old; 3. Radiological examination falls within the Pell & Gregory classification of types II to III, middle and low-position mandibular impacted wisdom teeth; 4. Clinical examination indicates that the extraction surgery requires incision and flap opening, bone removal to expose the wisdom teeth.

排除标准：

应排除以下患者： 1.患有高血压、心血管疾病、糖尿病者； 2.目前正在使用中枢神经系统抑制剂或镇痛药者； 3.有吸烟史者； 4.对麻醉剂或处方药物过敏者； 5.处于妊娠或哺乳期者； 6.以及存在任何可能影响愈合预后情况者； 6.手术时间超过 45 分钟的患者也需排除。

Exclusion criteria：

The following patients should be excluded: 1. Those with hypertension, cardiovascular diseases, or diabetes; 2. Those currently using central nervous system depressants or analgesics; 3. Those with a history of smoking; 4. Those allergic to anesthetics or prescription drugs; 5. Those who are pregnant or breastfeeding; 6. Those with any conditions that may affect the prognosis of healing; 6. Patients with a surgical duration of more than 45 minutes should also be excluded.

计划收集 40例下颌阻生第三磨牙拔除的患者，每组各20例。分组方案如下：项目实验员使用计算机程序生成 40 个随机序列，包含试验组（T）和对照组（C），如：T、C、C、T、C、T、T、C、T、C……；制备不透光、编号的随机信封（共 40 个），按序列将分组结果装入对应编号信封。患者就诊经筛选符合纳入标准后，按就诊顺序分配编号（1-40）。研究护士向项目实验员申请对应编号的随机信封，当场拆封，记录分组结果。手术医生仅知晓患者编号，按微创拔牙标准操作，不参与分组和头套佩戴。最后由研究护士根据分组情况决定是否为患者佩戴加压头套。

Randomization Procedure (please state who generates the random number sequence and by what method)：

A total of 40 patients undergoing extraction of impacted mandibular third molars will be enrolled, with 20 patients in each group. The grouping protocol is as follows: Researchers will use a computer program to generate 40 random sequences, which include the test group (T) and the control group (C). For example: T, C, C, T, C, T, T, C, T, C... Prepare 40 opaque, numbered random envelopes. Place the grouping results into the envelopes corresponding to their numbers according to the generated sequences. After patients visit the clinic and are screened to meet the inclusion criteria, they will be assigned numbers (ranging from 1 to 40) in the order of their visits. The research nurse will request the random envelope with the corresponding number from the researchers, open the envelope on the spot, and record the grouping result. The operating surgeon will only be aware of the patient's number, perform the procedure in accordance with the standard minimally invasive tooth extraction protocol, and will not be involved in grouping or the wearing of pressure headgear. Finally, the research nurse will decide whether to have the patient wear the pressure headgear based on the grouping result.

现有方案明确 “手术医生单盲”，但研究护士知晓分组，手术医生不知晓分组情况，且由同一医师实施微创拔牙，能最大程度保证两组手术操作的一致性，避免因医生知晓分组导致的操作差异。

Blinding：

The existing protocol clearly specifies "single-blinding of the operating surgeon": while research nurses are aware of the group assignments, the operating surgeon remains blinded. Furthermore, minimally invasive tooth extraction is performed by the same physician, which maximizes the consistency of surgical procedures between the two groups and avoids operational discrepancies arising from the surgeon's knowledge of group allocations.

本研究采用结构化病例记录表（CRF），涵盖受试者基本信息、诊疗过程、观察指标等核心模块，用于规范记录临床数据；电子数据采集和管理使用 ResMan 系统（基于互联网的 EDC），实现数据的电子录入、核查、存储与溯源，保障数据的准确性、完整性与可追溯性，同时实现 CRF 与 EDC 的联动管理，提升数据管理效率。

In this study, a structured Case Report Form (CRF) was adopted, covering core modules such as subjects' basic information, diagnosis and treatment process, and observation indicators, to standardize the recording of clinical data. For electronic data collection and management, the ResMan system (an internet-based Electronic Data Capture, EDC) was utilized to enable electronic data entry, verification, storage, and traceability. This system ensures data accuracy, completeness, and traceability while achieving integrated management of CRF and EDC, thereby improving data management efficiency.
