研究目的：

本研究通过前瞻性诊断性试验设计，系统性地评价超细胃镜在80岁以上患者上消化道早癌筛查中的应用价值与安全性。研究将精确评估其诊断效能（灵敏度、特异度等），全面分析其临床安全性（不良事件发生率），并深入探究影响患者接受度与完成率的关键因素。最终，旨在基于研究证据，建立一套科学、规范、适合中国国情的超细胃镜在高龄患者上消化道早癌筛查中的应用路径与操作规范，为优化我国高龄人群上消化道早癌防控策略提供高级别的循证医学证据。  

Objectives of Study：

This study systematically evaluated the application value and safety of ultrafine gastroscopy in screening for early upper gastrointestinal cancer in patients over 80 years old through a prospective diagnostic trial design. The study will accurately evaluate its diagnostic efficacy (sensitivity, specificity, etc.), comprehensively analyze its clinical safety (incidence of adverse events), and deeply explore the key factors that affect patient acceptance and completion rates. Ultimately, based on research evidence, the aim is to establish a scientific, standardized, and suitable application path and operational norms for ultrafine gastroscopy in screening for early upper gastrointestinal cancer in elderly patients, providing high-level evidence-based medicine for optimizing the prevention and control strategies of early upper gastrointestinal cancer in China's elderly population.

纳入标准：

1.年龄≥80岁，性别不限制；
2.合并上消化道症状（如吞咽困难、腹痛、消瘦等）或高危因素（如幽门螺杆菌感染、慢性萎缩性胃炎病史、胃癌家族史等）拟接受胃镜筛查；
3.同意采集研究所需的人口统计学及临床数据；
4.同意参加研究并签署书面知情同意书；

Inclusion criteria

1.Age >= 80 years old, gender not limited; 2.Combine upper gastrointestinal symptoms (such as swallowing difficulties, abdominal pain, weight loss, etc.) or high-risk factors (such as Helicobacter pylori infection, history of chronic atrophic gastritis, family history of gastric cancer, etc.) to undergo gastroscopy screening; 3.Agree to collect demographic and clinical data required for the study; 4.Agree to participate in the study and sign a written informed consent form;

排除标准：

1.既往有过任何上消化道手术史； 2.合并严重心肺、血液、免疫系统疾病； 3.合并其他实体瘤或血液肿瘤； 4.存在胃镜检查绝对禁忌证（如消化道穿孔等）； 5.年龄大于 100 周岁。

Exclusion criteria：

1.Have any previous history of upper gastrointestinal surgery; 2.Merge severe cardiovascular, hematological, and immune system diseases; 3.Merge other solid tumors or hematological tumors; 4.There are absolute contraindications for gastroscopy examination (such as gastrointestinal perforation, etc.); 5. Over 100 years old.
