研究目的：

围手术期多种因素参与了术后 ALI的发生，除了缺血再灌注损伤外，术中大量输血、机械通气、液体超载、氧化应激反应等因素均可引发ALI，虽然体外生命技术可有效改善患者术中氧合、维持血流动力学稳定，但随着移植肺的开放、ECMO流量的降低和停机，极易发生肺缺血-再灌注损伤，并可能通过损伤细胞和上调促炎因子水平两种途径，进一步诱导炎症反应从而导致ALI。因此，为了更精准地预防围术期急性肺损伤的发生，提高术后生存率，我们有必要对接受肺移植的患者进行回顾性分析，探究移植围术期急性肺损伤的发生机制以及如何进行有效的肺保护。  

Objectives of Study：

Perioperative factors are involved in the occurrence of postoperative ALI. In addition to ischemia-reperfusion injury, intraoperative massive blood transfusion, mechanical ventilation, fluid overload, oxidative stress reaction and other factors can cause ALI. Although extracorporeal life technology can effectively improve intraoperative oxygenation of patients and maintain hemodynamic stability, with the opening of transplanted lungs, the decrease of ECMO flow and shutdown, the risk of postoperative ALI can be reduced. Lung ischemia-reperfusion injury is very easy to occur, and may further induce inflammatory response through two ways of damaging cells and upregating pro-inflammatory factors, leading to ALI. Therefore, in order to more accurately prevent the occurrence of perioperative acute lung injury and improve postoperative survival rate, it is necessary to conduct retrospective analysis of patients receiving lung transplantation to explore the occurrence mechanism of perioperative acute lung injury and how to carry out effective lung protection.

纳入标准：

2019 年 1 月-2025 年 12 月在无锡市人民医院接受肺移植的患者

Inclusion criteria

Patients who received lung transplants at Wuxi People's Hospital from January 2019 to December 2025

排除标准：

1.患者有严重肾功能不全； 2.严重心脏功能不全者排除在外

Exclusion criteria：

1. The patient has severe kidney dysfunction; 2. Those with severe heart dysfunction are excluded
