研究目的:

1.通过1?F-AlF-NOTA-FAPI(1?F-FAPI)成纤维细胞活化显像联合 1?F-FDG PET/CT全身炎症显像动态(急性期、慢性期)评价AD(夹层、壁间血肿、溃疡)患者的血管炎症-纤维化过程,探索其潜在的病理生理学机制; 2.通过病理组织学检测以及血清学相关指标,进一步探索AD患者的分子机制,深入临床对疾病的认识,以及为AD患者的治疗提供新靶点; 3.本研究拟通过后续的随访工作(术后1个月、半年、1年复查主动脉CTA)获得预后信息,随访主动脉及支架相关不良事件。分析不同时期的FDG及FAPI的摄取活性对AD患者预后的影响,旨在进行危险分层,明确不同核素显像参数对AD的预后价值,有助于临床筛选高危人群,辅助临床治疗决策的制定。  

Objectives of Study:

1. By combining 1?F-AlF-NOTA-FAPI fibroblast activation imaging with 1?F-FDG PET/CT whole-body inflammation imaging (acute and chronic phases) to dynamically evaluate the vascular inflammation-fibrosis process in patients with AD (aneurysm, intramural hematoma, ulcer), we aim to explore the potential pathophysiological mechanisms; 2. Through pathological histological examination and serological related indicators, we further explore the molecular mechanism of AD patients, deepen the clinical understanding of the disease, and provide new targets for the treatment of AD patients; 3. This study intends to obtain prognostic information through subsequent follow-up work (rechecking aortic CTA at 1 month, half a year, and 1 year after surgery) and follow up on aortic and stent-related adverse events. Analyze the impact of FDG and FAPI uptake activity at different periods on the prognosis of AD patients, aiming to conduct risk stratification, clarify the prognostic value of different radionuclide imaging parameters for AD, and help clinical screening of high-risk populations and assist in the formulation of clinical treatment decisions.

纳入标准:

1.前瞻性纳入2024年11月至2025年7月于我科拟行手术治疗的确诊为AD的患者 2.年龄>=18周岁 3.术前3个月内行主动脉CTA及1?F-FDG和1?F-FAPI PET/CT显像 4.可以配合后续随访工作

Inclusion criteria

1. Prospective inclusion of patients diagnosed with AD and scheduled for surgical treatment in our department from November 2024 to July 2025. 2. Age >= 18 years old. 3. Had undergone aortic CTA and 1?F-FDG and 1?F-FAPI PET/CT imaging within 3 months prior to the surgery. 4. Can cooperate with the subsequent follow-up work.

排除标准:

1.围术期死亡 2.创伤性夹层 3.马凡综合征等遗传结缔组织病所导致的夹层 4.图像质量较差

Exclusion criteria:

1. Perioperative death 2.Traumatic dissection 3.Dissection caused by genetic connective tissue diseases such as Marfan syndrome 4. Poor image quality
