研究目的：

探讨提肛运动对严重慢性阻塞性肺疾病(COPD)患者运动耐受性和生活质量的影响。  

Objectives of Study：

To investigate the efficacy of levator ani exercise on exercise tolerance and quality of life in patients with severe chronic obstructive pulmonary disease (COPD).

药物成份或治疗方案详述：

稳定药物治疗:ICS/LBA (SymbicortTurbuhalerb):医生教患者正确使用吸入装置，布地奈德和富马酸福莫特罗粉剂吸入160ug或320ug一天两次，白三烯受体拮抗剂(Singulairc):孟鲁司特钠片10mg一晚一次。 提肛运动训练方法[18][19]:患者坐下，双脚并拢，双手叉起，然后站起来，缓慢吸气，同时腹肌收缩，肛门提肛，坚持5秒钟。逐步延长肛门提肛时间，每天早晨、中、晚做提肛运动，每次5-10分钟。 

Description for medicine or protocol of treatment in detail：

Stable drug therapy: ICS/LBA (SymbicortTurbuhalerb):The doctor teach patients to use inhaling device correctly,Budesonide and Formoterol Fumarate Powder for Inhalation 160ug or 320ug twice a day, leukotriene receptor antagonist (Singulairc): Montelukast Sodium Tablets 10mg once a night. levator ani exercise training method[18][19]: the patient sit down,and his feet close together, hands forke,then stand up, slowly inhale, at the same time his abdominal muscle contract, and anus lift,hold on for five seconds. gradually prolong anus lifting time, Every morning, middle, late to do levator ani exercise, each time 5-10 minutes.  

纳入标准：

FEV1占估计值的30%-50%。患者年龄40-70岁，小学及以上学历，沟通无障碍。

Inclusion criteria

Patients with severe COPD at stable stage; FEV1 accounts for 30%-50% of the estimated value; Aged 40-70 years old; primary school education or above, communication barrier-free.

排除标准：

1.肺大泡严重，肋骨骨折，气胸不引流;
2.有严重的心肺疾病、肺性脑病等严重并发症的;
3.进行其他临床试验的;
4.遗传性过敏、哮喘、变应性鼻炎。
病例剔除标准:
(1)入组后发现入组错误或误诊;
(2)研究期间未按照研究计划进行治疗的;
(3)研究期间急性加重;
(4)因其他疾病中断治疗。

Exclusion criteria：

Case exclusion criteria: (1) severe lung bullae, rib fracture, pneumothorax without drainage; (2) suffering from serious complications, such as severe cardiopneumatic disease, pulmonary encephalopathy; (3) those undergoing other clinical trials; (4) hereditary allergies, asthma, allergic rhinitis. Case rejection criteria: (1) after the subjects were enrolled, it was found that the patients were wrongly included or misdiagnosed; (2) those who were not treated according to the research plan during the study period; (3) acute aggravation during the study; (4) those who interrupted the treatment due to other diseases.
