研究目的：

帕金森病(PD)是一种具有较高神经精神症状伴发的神经退行性疾病，这些精神症状会在 PD发展过程中加重临床表现。然而，抑郁在日常临床实践中尚未被充分识别为 PD 的合并症，多达 50% 的 PD 患者未被诊断出合并抑郁。结构影像表明，PD 患者的额叶萎缩增加与 PD 合并抑郁等神经精神症状密切相关，然而功能影像的变化研究甚少。因此，充分认识 PD患者发生抑郁的相关因素，结合影像学生物标志物，提高 PD 合并抑郁的诊断率，对改善 PD 患者的生活质量具有重要意义。在本研究中，我们拟通过严格筛选纳入12名帕金森病伴抑郁患者，采集他们的18F-FDG PET脑部葡萄糖代谢图像作为样本进行脑网络分析，先找出STN-DBS对伴不同程度抑郁的帕金森病脑葡萄糖代谢差异的脑区，进而阐明STN-DBS对伴不同程度抑郁的帕金森病脑葡萄糖代谢神经网络的影响。  

Objectives of Study：

Parkinson's disease (PD) is a neurodegenerative disorder frequently complicated by neuropsychiatric comorbidities that exacerbate clinical progression. Depression remains critically underdiagnosed in PD, with nearly 50% of cases unrecognized clinically. While structural imaging links frontal atrophy to PD-associated depression, functional neuroimaging correlates remain poorly defined. Addressing this gap requires integrated analysis of depression-related factors and neuroimaging biomarkers to optimize diagnostic accuracy and clinical outcomes.This study will analyze cerebral glucose metabolism using 18F-FDG PET in 12 carefully selected PD patients with depression. Through brain network analysis, we aim to: (1) identify STN-DBS-induced metabolic differences across depression severity strata, and (2) characterize STN-DBS effects on glucose metabolic networks in depressed PD patients.

纳入标准：

1.符合2015年国际帕金森病及运动障碍学会(MDS)原发性PD的诊断标准； 2.Hoehn?Yahr分期 >= 2.5级； 3.既往无严重的神经和精神疾病史，无使用抗抑郁药、其他抗精神病药物以及无长期使用助眠药物史； 4.所有患者均行STN?DBS治疗

Inclusion criteria

1. Meets the diagnostic criteria of primary Parkinson's disease as defined by the International Parkinson and Movement Disorder Society (MDS) in 2015; 2. Hoehn-Yahr stage >= 2.5; 3. Has no history of severe neurological or mental disorders, no history of using antidepressants, other antipsychotic drugs, or long-term use of sleep aids; 4. All patients underwent STN-DBS treatment.

排除标准：

1.其他因素导致的帕金森综合征； 2.脑外伤、脑卒中或脑部手术等脑部疾病病史； 3.严重焦虑、失眠、抑郁和精神分裂症等精神疾病病史； 4.诊断PD前明确诊断有抑郁的患者； 5.有抑郁家族史者； 6.合并其他系统严重疾病病史； 7.空腹血糖＞6.1mmol/L或＜3.9mmol/L

Exclusion criteria：

1. Parkinson's syndrome caused by other factors; 2. History of brain diseases such as brain trauma, stroke or brain surgery; 3. History of mental disorders such as severe anxiety, insomnia, depression and schizophrenia; 4. Patients who were clearly diagnosed with depression before the diagnosis of PD; 5. Those with a family history of depression; 6. Those with a history of severe diseases in other systems; 7. Fasting blood glucose > 6.1 mmol/L or < 3.9 mmol/L
