本研究旨在探讨在AI辅助系统下提高NERD检出率的方法，降低患者的医疗和非医疗费用，为临床诊疗中提供一种创新的辅助诊断手段。同时，本研究也旨在推动医学和工学领域的交叉和融合，达到消化系统疾病的早诊和早治，减轻公共卫生的经济负担。  

Objectives of Study：

This study aims to explore methods to improve the detection rate of NERD with the assistance of AI-aided systems, reduce patients' medical and non-medical costs, and provide an innovative auxiliary diagnostic tool for clinical diagnosis and treatment. Meanwhile, it also seeks to promote the intersection and integration of medical and engineering fields, achieve early diagnosis and treatment of digestive system diseases, and alleviate the economic burden of public health.

纳入标准：

1.第一部分探讨 NERD 的不同特征性变化与反流的相关性，其研究对象来源于2018年-2026 年于遂宁市中心医院诊断为 NERD，具有胃镜、高分辨率食管动力检测以及反流监测资料的患者 纳入标准：确诊为 NERD 的患者，具有完整的胃镜、高分辨食管动力检测以及反流监测（24 小时 pH 监测或 24 小时 pH 联合阻抗监测）资料。NERD 的诊断表现：长病程（>6 月），具有反流相关症状，内镜下不符合反流性食管炎的洛杉矶分级，酸暴露时间>4.0%。 2.研究第二部分建立 AI 辅助诊断 NERD 的模型，其研究对象来源于2018 年至2026年遂宁市中心医院消化内镜中心的影像资料。 纳入标准：符合上消化内镜检查规范的胃食管结合部图像。 3.研究第三部分验证模型的诊断效能，其研究对象来源于研究第一部分以及第二部分完成后的遂宁市中心医院消化内镜中心的影像资料。纳入标准：>18 岁，因筛查、胃肠道症状调查或监测而转诊为上消化道内窥镜检查的患者。

Inclusion criteria

1. The first part explores the correlation between different characteristic changes of NERD and reflux. The study subjects were patients diagnosed with NERD at Suining Central Hospital from 2018 to 2026, who had gastroscopy, high-resolution esophageal manometry, and reflux monitoring data. Inclusion criteria: patients diagnosed with NERD, with complete gastroscopy, high-resolution esophageal manometry, and reflux monitoring (24-hour pH monitoring or 24-hour pH combined impedance monitoring) data. Diagnostic criteria for NERD: long disease course (>6 months), reflux-related symptoms, endoscopy findings not corresponding to Los Angeles classification of reflux esophagitis, acid exposure time >4.0%. 2. The second part of the study establishes an AI-assisted NERD diagnostic model. The study subjects were images obtained from the Digestive Endoscopy Center of Suining Central Hospital from 2018 to 2026. Inclusion criteria: images of the gastroesophageal junction that meet upper gastrointestinal endoscopy standards. 3. The third part of the study verifies the diagnostic performance of the model. The study subjects were images obtained from the Digestive Endoscopy Center of Suining Central Hospital after completing parts one and two of the study. Inclusion criteria: patients over 18 years old, referred for upper gastrointestinal endoscopy for screening, gastrointestinal symptom investigation, or monitoring.

排除标准：

1: 研究第一部分排除标准：排除标准：胃食管结合部胃镜图片质量差，酸暴露时间<4.0%，反流食管炎，巴雷特食管，胃食管流出道梗阻障碍性疾病，嗜酸性食管炎，食管裂孔疝，上消化道手术史，食管恶性肿瘤。 2: 研究第二部分排除标准：排除标准：不符合上消化内镜检查规范的胃食管结合部图像 3: 研究第三部分排除标准：怀孕，食管恶性肿瘤，上消化道手术史，食道潴留，已知活动性上消化道出血或急诊内窥镜检查、食道狭窄或严重潜在疾病的患者，拒绝参与的患者。

Exclusion criteria：

1. Exclusion criteria for the first part of the study: Poor-quality gastroscopic images of the gastroesophageal junction (GEJ); acid exposure time (AET) < 4.0%; reflux esophagitis (RE); Barrett's esophagus (BE); gastroesophageal outflow obstruction (GOO); eosinophilic esophagitis (EoE); hiatal hernia; history of upper gastrointestinal surgery; malignant esophageal tumors. 2. Exclusion criteria for the second part of the study:Images of the gastroesophageal junction (GEJ) that do not meet the standards of upper gastrointestinal endoscopy. 3. Exclusion criteria for the third part of the study: Pregnancy; malignant esophageal tumors; history of upper gastrointestinal surgery; esophageal stasis; patients with known active upper gastrointestinal bleeding, those undergoing emergency endoscopy, patients with esophageal stricture or severe underlying diseases; patients who refuse to participate.
