研究目的：

主要研究目的：探讨免疫治疗对于颅神经受侵鼻咽癌患者神经功能恢复的影响，明确免疫治疗时代颅神经恢复的动态规律及关键时间节点。 次要研究目的：分析不同颅神经（三叉神经、外展神经、面神经、舌下神经等）受侵患者的功能恢复差异；评估颅神经恢复与肿瘤预后（总生存期、无进展生存期、无局部区域复发生存期、无远处转移生存期）的相关性；探索免疫治疗期间颅神经功能早期变化对最终恢复状态的预测价值；分析诱导化疗、同期化疗等治疗因素与免疫治疗的交互作用对神经功能恢复的影响。  

Objectives of Study：

Primary Objective:to investigate the impact of immunotherapy on neurological function recovery in nasopharyngeal carcinoma patients with cranial nerve invasion, and to elucidate the dynamic patterns and critical time nodes of cranial nerve recovery in the immunotherapy era.Secondary Objectives:to analyze functional recovery disparities among patients with involvement of distinct cranial nerves (trigeminal nerve, abducens nerve, facial nerve, hypoglossal nerve, etc.);To evaluate the correlation between cranial nerve recovery and oncological outcomes, including overall survival (OS), progression-free survival (PFS), locoregional relapse-free survival (LRRFS), and distant metastasis-free survival (DMFS);To explore the predictive value of early neurological changes during immunotherapy for ultimate functional recovery status;To analyze the interactive effects between immunotherapy and therapeutic factors (induction chemotherapy, concurrent chemotherapy, etc.) on neurological function recovery.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.经鼻咽部肿物活检病理学检查确诊为鼻咽癌；
2.根据美国癌症联合委员会（AJCC）第9版分期标准，临床分期为T4期，且伴有颅神经受侵症状；
3.拟接受以免疫治疗、化学治疗及放射治疗为基础的综合治疗；
4.能够配合完成各项检查，并同意接受定期随访；

Inclusion criteria

1. Histologically confirmed nasopharyngeal carcinoma (NPC) by biopsy of nasopharyngeal mass. 2. Clinical stage T4 according to the 8th edition of the American Joint Committee on Cancer (AJCC) staging system, with concurrent cranial nerve involvement. 3. Planned to receive comprehensive treatment comprising immunotherapy combined with chemotherapy and radiotherapy. 4. Ability to comply with the prescribed examinations and willingness to undergo regular follow-up assessments.

排除标准：

1.既往已接受过其他头颈部放射治疗者；
2.初次诊断时合并其他部位原发恶性肿瘤；
3.既往有糖尿病、多发性硬化、吉兰-巴雷综合征或其他可能影响颅神经功能的神经系统疾病；
4.既往有颈部外伤或手术史导致颅神经损伤者；

Exclusion criteria：

1. History of previous radiotherapy to the head and neck region. 2. Synchronous primary malignancy at other anatomical sites at the time of initial diagnosis. 3. Pre-existing diabetes mellitus, multiple sclerosis, Guillain-Barré syndrome, or other neurological disorders that may affect cranial nerve function. 4. History of neck trauma or surgical procedures resulting in cranial nerve damage.
