研究目的：

开展多中心、开放标签、盲法终点评估的随机对照临床试验，明确急性非大动脉闭塞致残性卒中患者早期使用地塞米松磷酸钠的有效性及安全性。  

Objectives of Study：

Conducted a multicenter, open-label, randomized controlled clinical trial with blinded endpoint assessment to determine the efficacy and safety of early administration of dexamethasone sodium phosphate in patients with acute, disabling non–major-artery occlusion ischemic stroke.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄>=18 周岁，不设年龄上限； 2.根据临床症状或影像检查初步判断为急性缺血性脑卒中； 3.发病至随机化在 24h 内； 4.根据 CTA/MRA/DSA 确定为非大动脉闭塞性卒中； 5.入组前 NIHSS 评分>5 分， 或存在致残性症状（如孤立性失语、偏盲或肢 体无力影响功能） ； 6.近一个月无出血史（消化系统及泌尿系统出血）； 7.患者或家属签署书面知情同意书。

Inclusion criteria

1.Age >=18 years old, with no upper age limit; 2.Acute ischemic stroke diagnosed based on clinical symptoms or imaging findings; 3.Randomization within 24 hours of symptom onset; 4.Non-large artery occlusive stroke confirmed by CTA/MRA/DSA; 5.NIHSS score > 5 points before enrollment, or presence of disabling symptoms (such as isolated aphasia, hemiparesis, or sensory deficits); 6.No history of bleeding in the past month (excluding gastrointestinal and urinary tract bleeding); 7.Patient or family member signed informed consent form;

排除标准：

1.经头颅 CT 或 MRI 证实为颅内出血；
2.发病前 mRS 评分≥2 分；
3.妊娠或哺乳期妇女；
4.计划进行或已进行急性期血管内治疗；
5.本次发病入组前正在使用糖皮质激素类药物；
6.近一月有出血史（胃肠道及尿路出血） ；
7.慢性血液透析及严重肾功能不全（肾小球滤过率<30ml/min 或血肌 酐>220μmol/L）患者；
8.任何疾病晚期致预期寿命<6 个月；
9.预期不能完成随访；
10.对糖皮质激素、 亚硫酸盐或造影剂过敏；
11.正参加其他可能影响本试验结果的临床试验；
12.黄斑水肿；
13.全身感染性疾病；
14.莫氏Ⅱ型二度/三度房室传导阻滞或病态窦房结综合征或病史。

Exclusion criteria：

1.Intracranial hemorrhage confirmed by cranial CT or MRI;
2.Pre-morbid mRS score ≥2;
3.Pregnant or lactating women;
4.Planned or undergone acute endovascular therapy;
5.Currently using corticosteroids before enrollment in this episode;
6.Bleeding history in the past month (gastrointestinal or urinary tract bleeding);
7.Severe renal dysfunction on chronic hemodialysis (glomerular filtration rate <30ml/min or creatinine ≥220μm);
8.Any disease with expected life expectancy <6 months;
9.Expected inability to complete follow-up;
10.Allergy to corticosteroids, sulfites, or contrast agents;
11.Currently participating in other clinical trials that may affect the results of this trial;
12.Jaundice and edema;
13.Systemic infectious disease;
14.Mobitz Type II second/third-degree atrioventricular block or atrial fibrillation syndrome or history thereof;
