研究目的: |
主要研究目的: 评估在标准化营养支持基础上联合12周低强度高频振动训练对老年肌少症患者肌肉质量和肌肉力量的改善效果,具体指标为:握力的绝对变化值 次要研究目的: 1. 评估联合干预方案对患者肌肉质量的改善效果:四肢骨骼肌指数(ASMI)的绝对变化值; 2. 评估联合干预方案对患者躯体功能的影响:简易机体功能评估(SPPB)总分变化,5次坐站测试(5-TCS)完成时间变化,6米日常步速变化; 3. 分析联合干预对人体成分的影响:BIA测量的体重、BMI、去脂体重、骨骼肌质量等指标变化,体脂肪量、体脂百分比、内脏脂肪面积变化,体液分布及节段肌肉/脂肪质量变化。 4. 探索联合干预的生物学机制:肌肉合成通路标志物(IGF-1、IGFBP-3、Irisin、Myostatin)水平变化;骨代谢标志物(PINP、β-CTX、骨钙素)水平变化;内分泌及代谢指标(性激素、PTH、25-羟基维生素D)水平变化; 5. 评估对老年综合评估及生活质量的影响:微型营养评估短问卷(MNA-SF)、Frail衰弱量表、MORSE跌倒评估表、Berg平衡量表、焦虑症筛查自评量表(GAD-7)、抑郁症筛查自评量表(PHQ-9)、简易智能精神状态检查量表(MMSE)、匹兹堡睡眠质量指数量表(PSQI)、基本日常生活活动能力评估量表(Barthel)、工具性日常生活活动能力评估量表(IADL)、老年疾病累积评估量表(CIRS-G)的评分变化。 6. 表面肌电图记录目标肌群的活动。 7. 近红外脑功能成像仪(fNIRS)检测大脑活动变化。 8. 观察长期疗效与安全性:干预停止后12周的效果维持情况,不良事件发生率及安全性评价。 |

Objectives of Study: |
PRO:Evaluate efficacy of 12-week low-intensity high-frequency WBV training + nutritional support vs. nutritional support alone for improving handgrip strength in elderly with sarcopenia.SROs:1.Assess effect on muscle mass via ASMI change.2.Evaluate impact on physical function via SPPB score, 5-TCS time, 6-meter walk speed.3.Analyze effects on body composition via BIA-derived parameters (e.g., BMI, fat-free mass, fat mass, visceral fat).4.Explore mechanisms via serum changes in: IGF-1, IGFBP-3, irisin, myostatin; PINP, β-CTX, osteocalcin; sex hormones, PTH, 25-hydroxyvitamin D.5.Assess geriatric and QoL outcomes via MNA-SF, FRAIL, Morse, Berg, GAD-7, PHQ-9, MMSE, PSQI, Barthel (ADL), IADL, CIRS-G.6.Record muscle activity via surface EMG.7.Detect brain activity changes via fNIRS.8.Observe long-term efficacy/safety: 12-week post-intervention effect maintenance, AE incidence, overall safety. |

Inclusion criteria |
1. Age >= 65 years, any gender; 2. Meet the diagnostic criteria of the 2025 Asian Working Group for Sarcopenia (AWGS) Expert Consensus, specifically fulfilling both conditions: 1) Muscle strength reduction: handgrip strength < 28 kg for males and < 18 kg for females; 2) Low muscle mass: Appendicular Skeletal Muscle Index (ASMI) assessed by Dual-energy X-ray Absorptiometry (DXA) < 7.0 kg/m2 for males and < 5.4 kg/m2 for females. 3. Be lucid and possess basic communication skills, capable of understanding and willing to comply with the study procedures. 4. Voluntarily participate in this study and have provided written informed consent. |

排除标准: |
1.存在振动训练禁忌症,包括但不限于:严重心律失常、未控制的高血压、近期深静脉血栓、体内有不稳定金属植入物或心脏起搏器携带者。 2.患有严重关节疾病(如晚期膝骨关节炎)、神经系统疾病(如帕金森病、卒中后严重偏瘫),导致无法完成训练动作。 3.合并严重骨质疏松或近期有椎体、髋部等脆性骨折史,存在较高骨折风险。 4.患有严重或不稳定的心、肝、肾、肺等重要脏器功能衰竭,或恶性肿瘤终末期,预期生存期<1年。 5.正在规律参与其他结构化的抗阻或有氧运动训练计划,或近3个月内参与过其他干预性临床研究。 6. 研究者判断存在任何其他不适宜参加本研究的医学或心理状况。 |

Exclusion criteria: |
1. Presence of contraindications to whole-body vibration training, including but not limited to: severe arrhythmia, uncontrolled hypertension, recent deep vein thrombosis, individuals with unstable metal implants or implanted pacemakers. 2. Have severe joint diseases (e.g., advanced knee osteoarthritis) or neurological disorders (e.g., Parkinson's disease, severe hemiplegia after stroke) that prevent completion of the training movements. 3. Have severe osteoporosis or a recent history of fragility fractures (e.g., vertebral or hip fractures), presenting a high risk of fracture. 4. Have severe or unstable failure of vital organs (heart, liver, kidney, lungs, etc.) or terminal malignant tumor with an expected survival of <1 year. 5. Are currently regularly participating in other structured resistance or aerobic exercise training programs, or have participated in other interventional clinical studies within the past 3 months. 6. Have any other medical or psychological condition deemed by the investigator as unsuitable for participation in this study. |
