研究目的：

本研究采用上下设计的前瞻性序贯试验来评估右转方向和后/前屈方向的安全头部位置角度，在该角度下，90%的患者可以完成紧急气管插管。  

Objectives of Study：

This study used a prospective sequential trial with an up and down design to evaluate the safe head position angles for both the turning-right direction and the backward/forward flexion direction, at which 90% of patients can complete emergency endotracheal intubation.

纳入标准：

----全麻下气管插管择期手术 ----年龄>=18岁，<=65岁，男女不限 ----ASA分类1-2 ----体重指数小于30kg/m2 ----研究参与者签署知情同意书

Inclusion criteria

——Selective surgery under general anesthesia with endotracheal intubation ——Age >= 18 years old, <= 65 years old, male or female not limited ——ASA classification 1-2 ——BMI less than 30kg/m2 ——Research participants sign informed consent forms

排除标准：

----术前评估为气道困难（mallampati 3-4级） ----头颈部、口腔科、五官科、咽喉部手术 ----头颈损伤或活动度异常 ----带可摘假牙和活动牙

Exclusion criteria：

——Preoperative assessment as difficult airway (mallampati grading 3-4) ——Head and neck, oral, facial features, and throat surgeries ——Head and neck injury or abnormal head and neck mobility ——With removable dentures and movable teeth

从中立位开始，将第一个参与者的头部向右旋转50度（通常，头部向右旋转的角度可以是60°~80°），根据插管成功与否决定下一例参与者的头部位置。如果插管失败，下一例参与者头部右旋角度将以10°的间隔减小；如果插管成功，保持这个角度，成功插管K次（ED90计算K=6）后，参与者头部右旋角度提高10°。以这种方式执行序贯试验，直到完成给定数量的样本案例。

From the neutral position, we turn the head 50 degrees to the right for the first participant (normally, the angle of head rotation to the right can be 60 °~80 °). The head position of the next participant was determined based on whether the intubation is successful or not. If intubation fails, the head position angle will decrease to the lower level in 10 ° intervals; After successful intubation for K times (K=6 for ED90 calculation), the head position angle was raised to the higher level. Perform sequential testing in this way until the given number of sample cases is completed.
