研究目的：

通过建立子宫切口憩室手术与未手术患者回顾性临床队列，收集患者术前以及术后临床结构化数据、影像学图像、影像学报告，采用深度学习视觉语言模型融合多模态数据构建CSD精准诊疗与预后预测的量化评估模型，为提升CSD的治疗水平与患者生育潜能提供坚实的科学依据与关键技术支撑。  

Objectives of Study：

By establishing a retrospective clinical cohort of patients with uterine incision diverticulum surgery and those without surgery, we collected preoperative and postoperative clinical structured data, imaging images, and imaging reports of the patients. We used a deep learning visual language model to integrate multimodal data to construct a quantitative assessment model for the precise diagnosis and prognosis prediction of CSD, providing solid scientific basis and key technical support for improving the treatment level of CSD and the reproductive potential of patients.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 有剖宫产史，且术后＞3 个月；有或无临床症状；经超声学和核磁共振检查提示剖宫产瘢痕部位存在子宫肌层缺损、子宫壁龛，临床诊断为剖宫产后子宫切口憩室患者； 2. 于本中心影像学检查确诊为剖宫产后子宫切口憩室，并由高级职称医生评估后决定采取保守治疗或者经阴道修补术的患者； 3. 治疗前临床文本资料与影像学原始数据完整； 4. 手术治疗患者：须具备术后 3 个月、6 个月的超声复查资料，以及术后 6 个月的核磁共振复查资料。 5. 无严重内科疾病（重要脏器功能在正常范围）。 6. 无恶性肿瘤病史。

Inclusion criteria

1. History of cesarean section, and more than 3 months post-surgery; with or without clinical symptoms; ultrasound and MRI examinations indicate myometrial defect or uterine wall niche at the cesarean scar site, clinically diagnosed as patients with cesarean scar defect; 2. Patients who were diagnosed with cesarean scar defect by imaging at this center and, after assessment by senior physicians, decided to undergo conservative treatment or transvaginal repair surgery; 3. Complete pre-treatment clinical text data and original imaging data; 4. For surgical patients: must have ultrasound follow-up data at 3 months and 6 months post-surgery, and MRI follow-up data at 6 months post-surgery; 5. No serious internal medical conditions (vital organ functions within normal range); 6. No history of malignant tumors.

排除标准：

1.诊断标准不能完全满足，未满足影像学依据；
2.临床文本资料或影像学图像不完整，无法提取所需变量；
3.有严重内科疾病（严重的肝病、肾病、呼吸道疾病、心脏疾病或糖尿病、癫痫等，重要脏器功能异常）；
4.合并恶性肿瘤患者；
5.妊娠状态；
6.影像学图像质量不过关，包括严重模糊、序列不全等；

Exclusion criteria：

1.The diagnostic criteria were not fully met, and the imaging basis was not satisfied.
2.The clinical text data or imaging images are incomplete, making it impossible to extract the required variables.
3.Having severe internal medical conditions (severe liver disease, kidney disease, respiratory diseases, heart diseases or diabetes, epilepsy, etc., with abnormal functions of important organs);
4.Patients with malignant tumors combined.
5.Patients with regnancy state;
6.The quality of the imaging images is not satisfactory, including severe blurring and incomplete sequences, etc.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

病历记录表进行数据采集，研究人员按统一标准操作流程进行数据抽取。导出影像学原始数据（DICOM）后进行去标识化处理。文本数据提取后删除姓名、证件号、电话、住址等直接标识符，建立“研究编号—病案号/就诊号”对应表，仅由指定负责人在院内加密保存。采用EDC系统建立研究数据库，设置分级访问权限，仅授权研究团队成员访问。用于统计分析与模型训练/验证的数据均为去标识化数据。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

The case record form was used for data collection. The researchers carried out data extraction according to a unified standard operating procedure. After exporting the original imaging data (DICOM), de-identification processing was carried out. After extracting the text data, direct identifiers such as names, identification numbers, phone numbers, and addresses were deleted. An "research number - case number / visit number" correspondence table was established, and it was only encrypted and stored within the hospital by the designated person in charge. The EDC system was employed to establish the research database, and access permissions were set at different levels, with only the research team members being authorized to access it.
