Objectives of Study：

Prospective Comparison of Resection Rates and Cost-Effectiveness Analysis of Simplified Endoscopic Mucosal Resection Without Submucosal Injection Versus Endoscopic Submucosal Dissection for Rectal Neuroendocrine Tumors

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄18-80的成年受试者，性别不限； 2.经结肠镜发现考虑直肠神经内分泌肿瘤（直肠NET）且拟行内镜下治疗的患者； 3.内镜观察能基本判断病变局限于黏膜或黏膜下层，未见明显深部浸润表现，如内镜下难以判断，可由术者酌情行EUS； 4.病变最大直径<=10 mm（以内镜或EUS测量为准）； 5.受试者一般情况可耐受内镜治疗； 6.受试者或其法定代理人自愿参加本研究并签署书面知情同意书。

Inclusion criteria

1. Adult subjects aged 18-80 years old, regardless of gender; 2. Patients diagnosed with rectal neuroendocrine tumors (rectal NET) by colonoscopy and scheduled for endoscopic treatment; 3. The endoscopic examination can basically determine that the lesion is limited to the mucosa or submucosa, and no obvious deep infiltration is observed. If it is difficult to determine during the endoscopy, the operator can decide to perform EUS; 4. The maximum diameter of the lesion is <= 10 mm (measured by endoscopy or EUS); 5. The general condition of the subjects can tolerate endoscopic treatment; 6. The subjects or their legal representatives voluntarily participate in this study and sign a written informed consent form.

排除标准：

1.术前内镜评估病变最大直径>10mm，或内镜外观提示病变范围明显超出常规黏膜/黏膜下切除范围； 2.既往同一部位直肠NET已行内镜或外科切除，本次为残端/复发处理者； 3.内镜、EUS、或其他影像学、核医学检查提示可能累及固有肌层或有淋巴结明显肿大/转移倾向者； 4.合并同期其他消化道恶性肿瘤或需优先处理的高级别病变（如结直肠癌、进展期息肉等）； 5.出血高危而不能按规范停用抗凝/抗血小板药物者，如：需要持续使用双联抗血小板或华法林/DOAC且不能停药或桥接者； 6.明显凝血功能障碍（如INR>1.5，血小板<50×10^9/L）或活动性出血性疾病； 7.严重心、肺、肝、肾功能不全，无法耐受肠道准备或内镜治疗者； 8.妊娠或哺乳期女性； 9.合并炎症性肠病活动期、重度肛周疾病等影响直肠治疗或愈合的情况； 10.不能配合随访或预计随访期间将长期外出、无法完成研究计划的临床与内镜复查者； 11.研究者认为不适宜入组的其他情况

Exclusion criteria：

1. Preoperative endoscopic assessment shows that the maximum diameter of the lesion is greater than 10mm, or the endoscopic appearance indicates that the lesion range significantly exceeds the conventional mucosal/submucosal resection range; 2. The same site of rectal NET has undergone endoscopic or surgical resection in the past, and this is for the treatment of the stump/recurrence; 3. Endoscopy, EUS, or other imaging and nuclear medicine examinations suggest that it may involve the muscularis propria or have obvious lymph node enlargement/transfer tendency; 4. Concurrent other digestive tract malignancies or high-grade lesions (such as colorectal cancer, advanced polyps, etc.) that require priority treatment; 5. High-risk of bleeding and unable to stop anticoagulant/antiplatelet drugs as per the standard, such as those who need to continue using dual antiplatelet or warfarin/DOAC and cannot discontinue or bridge; 6. Severe coagulation dysfunction (such as INR > 1.5, platelet < 50×10^9/L) or active bleeding disorders; 7. Severe heart, lung, liver, or kidney dysfunction, unable to tolerate intestinal preparation or endoscopic treatment; 8. Pregnant or lactating women; 9. Complicated with active inflammatory bowel disease, severe perianal diseases, etc., which affect rectal treatment or healing; 10. Unable to cooperate with follow-up or expected to be long-term absent during the follow-up period and unable to complete the research plan and clinical and endoscopic reexamination; 11. Other situations that the investigator considers not suitable for inclusion in the study.
